摘 要:目的探讨体外实验条件下,辐照时间和血栓凝龄对超声波溶栓效率的影响。方法取健康人全血180ml,置于37℃恒温水浴孵育。取其中120ml均分为2,6,12,24,48,72h不同凝龄的体外血栓。每一凝龄的血栓等额分成辐照组与对照组,辐照组以2MHz、1.8W/cm2的脉冲超声波辐照10min;对照组予以假照。其余60ml凝龄时间为2h,等额分为辐照组和对照组,辐照组以2MHz、1.8W/cm2的脉冲超声波分别辐照20,30,60min;各对照组分别予以假照相同时间。比较辐照组与对照组之间的溶栓率,并分别计算溶栓率与凝龄、辐照时间之间的关系。结果凝龄2,6,12h的体外血栓在超声辐照10min条件下,溶栓率显著提高(P<0.05);超声辐照凝龄2h的体外血栓10,20,30,60min,溶栓率显著提高(P<0.01)。超声波体外溶栓效率与凝龄呈线性负相关(r1=-1.000,P<0.01),与辐照时间呈线性正相关(r2=1.000,P<0.01)。结论在体外实验条件下,2MHz、1.8W/cm2的脉冲超声波有明确的直接溶栓效果,其溶栓率与凝龄成反比,与辐照时间成正比。Objective To evaluate the effects of clotting time and insouation duration on the sonothrombolysis in vitro. Methotis One hundred and eighty thrombi samples of whole blood from healthy volunteers were prepared by water bath incubation at 37℃, 120 ml among which were divided into six groups equally and treated for various times of 2, 6, 12, 48 and 72 h. The thrombi of the same clotting time were divided into insouation and control groups. The thrombi in the insouation group were irradiated by pulse ultrasound of 2 MHz and 1.8 W/cm^2 for 10 min while the control was sham insonated. The other 60 samples prepared by incubation for 2 h were divided into 3 insouation groups and 3 controls equally. The insouation groups were irradiated by pulse ultrasound of 2 MHz and 1.8 W/cm^2 for 20, 30 and 60 min respectively and the control groups were dealt with sham insouation for the same time. The thrombolysis rates between the insouation groups and the controls were investigated and relationship between the thrombolysis and the clotting time or insouation time was calculated. Results The thrombolysis rate was improved in the thrombi clotting for 2 h, 6 h and 12 h with the ultrasound insouation of 10 min( P 〈 0.05)and in those clotting for 2 h with the insouation of 10 min, 20 min, 30 min and 60 min( P 〈 0.01 ). There was a negative correlation between the thrombolysis rate and the clotting time ( r1 = - 1. 000, P 〈 0.01 )and a positive correlation between it and the insouation time ( r2 = 1. 000, P 〈 0.01 ). Conclusion The pulse ultrasound of 2 MHz and 1.8 W/cm^2 has a direct thrombolysis effect in vitro and the thrombolysis rate is negatively correlated with the clotting time and positively correlated with the insouation time.
