颗粒脂肪注射治疗上睑凹陷畸形的并发症及其防治

摘　　要：目的探讨颗粒脂肪移植治疗上睑凹陷的并发症，以便引起重视。方法对72例上睑凹陷的患者在上睑凹陷部行颗粒脂肪注射移植后的并发症进行回顾性调查。其中8例是由于接受重睑成形术后导致的上睑凹陷，另外68例是原发性上睑凹陷。结果本组有17例共发生上睑不平整、上睑臃肿、上睑不对称、脂肪注入位置过浅、脂肪注入凹陷区外、穿刺针刺破泪腺和暂时性上睑下垂等7种并发症，并发症发生率为23．6％，其中9例发生了两种以上并发症。值得注意的是，本组有8例重睑术后上睑凹陷患者，出现了较高的并发症发生率，尤其是上睑不平整等现象。尽管这些患者出现了不同程度的并发症，但他们的上睑外观较手术前还是有明显改善。结论颗粒脂肪移植治疗上睑凹陷在经验不足或操作不当的情况下非常容易出现并发症，大多数并发症对面容有一定的负面影响，因此有效地防止出现并发症十分重要。Objective To investigate the complications induced by pellet fat injection for treating the patients suffering from sunken upper eyelid, so as to pay attention to and prevent them. Method The complicastions in 72 patients who receivied the pellet fat injection in their sunken upper eyelid were investigated after the operation. 8 of them were traumatic sunken upper eyelid caused by double eyelid operation, and the others were primary sunken upper eyelid. Results 7 kinds of complications emerged in 17 patients and more than two complications were found in 9 patients. The complications included smoothless, asymmetry, fatty, the pellet fat to be injected too shallow or out of the correct position, temporary ptosis and lachrymal gland stabbing. It was shown that there was higher complication morbidity after pellet fat injection in the sunken upper eyelid patients who had been received double eyelids operation before. Most of the patients were satisfactory to the results of the operation, although 23.6% complications occurred among them. Conclusions The complications of the fat injection for treating the sunken upper eyelid are very common if the doctor is unskillful, or the wrong performance could be done during the operation. So it is very important to prevent the complications effectively.
