结核专科急诊抢救病例226例临床分析

摘　　要：目的 了解结核专科医院急诊抢救病例的临床特点，提供培训依据。方法 选取我院2004年1月-2005年12月急诊抢救室收治的急危重患者226例进行回顾性分析。结果 226例患者中男168例，女58例；年龄1—90岁，其中≤20岁18例，≥60岁85例。抢救病因为：循环系统方面50例；呼吸系统方面133例（其中大咯血34例，张力性气胸40例等）；中枢神经系统疾病12例；中毒及其他系统疾病31例。基础病变为结核病的162例。结论结核急诊抢救病例临床特点为：1．病例分布各个年龄组，以中老年患者多见；2．病因以结核急症多见占71．7％，另涉及循环、呼吸、中枢神经等多个系统。因此，结核专科急诊要提高应急救治能力，要向全科方面发展。Objective To study the clinical symptoms of emergent cases in tuberculosis hospital, and to provide support for improving services in emergency center and clinical training to physicians. Methods 226 patients accepted to emergency center of our hospital from January 2004 to December 2005 were included in the study. Their clinical data were retrospectively analyzed. Results The study included 226 patients （ 168 man and 58 women） with the age ranging between 1 -90 years （ 18 patients aged ≤ 20 years, 63 patients aged 21 -40 years, 60 patients aged 41 -59 years, and 85 patients aged≥60 years）. Causes of disease for salvage were as follows： 50 cases of circulation system, 133 of respiration system（ including 34 heavy hemoptysis and 40 tension pneumothorax） , 12 of central nerve system disease, 5 intoxication and 31 other systemic diseases. Basic pathological changes of 162 cases attributed to tubereulosis. Conclusion Clinical symptoms of emergent cases in tuberculosis are summarized as follows. 1. Patient seen in emergency varied at different ages, with most seen in middle and senior age group. 2. Though tuberculosis has the highest rate for emergency treatment, multi systems are involved, including circulation, respiration and central nerve systems. 3. Basic pathological changes occur in most cases（71.7% ）, which attribute to development of tuberculosis. In order to enhance the effect of emergency treatment on those patients, tuberculosis department needs to further enhance all-round capability in dealing with the patients besides the professional advantage of tuberculosis specialty.
