摘  要:目的:探讨冠心病患者血管紧张素转化酶(ACE)基因多态性与血管活性物质间的关系。方法:提取冠心病患者血白细胞DNA,多聚酶链反应(PCR)检测ACE基因多态性,紫外吸收法检测血ACE活性,放免非平衡法测定血中血管活性物质[肾素(PRA)、血管紧张素Ⅱ(AngⅡ)、醛固酮(ALD)、内皮素(ET)]浓度,硝酸还原酶法测定一氧化氮(NO)浓度并做相关分析。结果:80例冠心病人ACE基因I/D多态性DD型27例,DI型39例,II型14例。正常对照组80例,DD型12例,DI型42例,II型26例。冠心病人DD型分布较正常对照组明显增高,II型分布显著减少,不同ACE基因型患者其血中ACE活性、AngⅡ、ALD、ET及NO浓度差异有显著性,相关分析发现冠心病组ACE与AngⅡ、ALD、ET浓度相关。结论:在研究人群中,ACE基因I/D多态性可能是预测冠心病危险性的遗传标志,ACE基因的这种多态性能显著地影响血清ACE水平,冠心病患儿ACE活性与AngⅡ、ALD、ET浓度相关。Objective To explore the relationships of polymorphism of angiotensin-eonverting enzyme (ACE) gene and vessel reactive substances in coronary heart disease (CHD) patients. Methods The polymorphism of ACE gene of leucocyte in the CHD patients was observed with polymerase chain reaction (PCR). The serum ACE activity was detected by ultra-violet spectrophotography. The vessel reactive substances including serum plasma renin activity (PRA), angiotensin Ⅱ (Ang Ⅱ) ,aldosterone(ALD) ,endothelin(ET) ,NO were measured by no-equilibration radioimmunassay. Results The I/D polymorphism of ACE gene was DD type 27, DI type 39 and Ⅱ type 14 in 80 CHD patients, DD type 12, DI type 42 and Ⅱ type 26 in healthy controls. The distribution of DD type in CHD patients was significantly higher while the distribution of II type was lower than that in healthy controls. The acitivities of CHD ACE, Ang Ⅱ , ALD, ET has a significant difference in different ACE gene patients. The activity of serum ACE had a positive correlation with levels of plasma Ang Ⅱ , ALD and ET, respectively. Conclusions The polymorphism of ACE gene may be the genetic mark that can probably predict to risk of CHD and influence the activity of ACE significantly. The latter has a positive correlation with serum levels of Ang Ⅱ , ALD and ET in CHD patients, respectively.
