摘  要:目的探讨白大衣性高血压及蒙面高血压患者的靶器官损害状况。方法100例研究对象分为健康对照组(A组)、白大衣性高血压组(B组)、蒙面高血压组(C组)及原发性高血压组(D组),检查各组的左室重量指数、颈动脉内膜厚度及检测尿β2微球蛋白、微量白蛋白水平并作比较。结果D组患者颈动脉内膜厚度及尿β2微球蛋白、微量白蛋白水平明显高于A组、B组、C组[颈动脉内膜厚度(mm):(0.7±0.1)vs(0.5±0.1),(0.6±0.1),(0.6±0.1);82微球蛋白(mg/L):(1.9±0.6)vs(0.8±0.1),(0.9±0.1),(1.3±0.1);微量白蛋白(mg/L):(43.4±7.2)vs(4.5±0.6),(4.7±0.6),(31.7±8.8);P〈0.01];C组患者颈动脉内膜厚度及尿β2微球蛋白、微量白蛋白水平显著高于A组及B组[颈动脉内膜厚度(mm):(0.6±0.1)vs(0.5±0.1),(0.6±0.1);β2微球蛋白(mg/L):(1.3±0.1)vs(0.8±0.1),(0.9±0.1);微量白蛋白(mg/L):(31、7±8.8)vs(4.5±0.6),(4.7±0.6);均P〈0.05];B组患者颈动脉内膜厚度(mm)显著高于A组[(0.6±0.1)vs(0.5±0.1);P〈0.01],但是两组间尿β2微球蛋白、微量白蛋白水平差异无统计学意义[(0.9±0.1)mm vs(0.8±0.1)mm;(4.7±0.6)mg/Lvs(4.5±0.6)mg/L;P〉0.05];B组、C组、D组左室重量指数均显著高于A组[(84.3±16.8)g/m^2,(91.0±18.7)g/m^2,(91.4±24.7)g/m^2 vs(71.6±18.4)g/m^2;均P〈0.05],但是他们三组之间差异无统计学意义。经多元线性回归分析:蒙面高血压患者白昼收缩压(SBP)水平与左室重量指数及尿微量白蛋白水平直线相关;白昼舒张压(DBP)水平与左室重量指数及尿β2微球蛋白、微量白蛋白水平直线相关。结论白�Objective To study the target organ damage in patients with white coat and masked hypertension. Methods The subjects were divided into four groups: normotensive control (NC) group (group A), white coat hypertension(WCH) group(group B),masked hypertension(MH) group(group C) and essential hypertension(EH) group(group D). The left ventricular mass index(LVMI),carotid intima-media thickness(IMT) and urinary β2-microglobulin(β2-MG) ,microalbumin(MA) were measured and compared to their levels in four groups. Results The levels of IMT,urinary β2-MG and MA in patients with EH were higher than those in patients with NC,WCH and MH[IMT:(0.7±0.1) vs (0.5±0.1),(0.6±0.1),(0.6±0.1);β2-MG:(1.9±0.6) vs (0.8±0.1),(0.9± 0.1),(1.3±0.1);MA:(43.4±7.2) vs (4.5±0.6),(4.7±0.6),(31.7±8.8);P〈0.01].ThelevelsoflMT, urinary β2-MG and MA in patients with MH were higher than those in patients with NC and WCH[IMT: (0.6 ± 0.1) vs(0.5±0.1),(0.6±0.1);β2-MG:(1.3±0.1) vs (0.8±0.1),(0.9±0.1);MA:(31.7±8.8) vs (4.5± 0.6), (4.7 ± 0.6) ; P 〈 0.05 ], and the levels of I MT in patients with WCH was higher than that in NC[ (0.6 ± 0.1 ) vs (0.5 ± 0.1 );P 〈 0.01 ], but no significant differences were found in urinary β2-MG and MA between these two groups[ (0.9 ± 0.1 ) vs (0.8 ± 0.1 ) ; (4.7 ± 0.6) vs (4.5 ± 0.6) ; P 〉 0.05 ]. The levels of LVMI in patients with WCH,MH,EH was higher than that in NC[ (84.3 ± 16.8) ,(91.0 ± 18.7) ,(91.4+ 24.7) vs (71.6± 18.4) ;P〈 0.05 ], but no significant differences were found among these three groups. On multipleregression analysis, the levels of SBP in patients with MH were lineally related to LVMI and urinary MA. The levels of DBP in patients with MH were lineally related to LVMI, urinary β2-MG and MA. Conclusion Both WCH and MH are associated with target organ damage.
