摘 要:目的探讨老年高血压患者胰岛素抵抗(IR)与血脂代谢的关系。方法将60例老年高血压患者根据有无合并糖尿病分为单纯高血压组30例(HP)组,高血压合并2型糖尿病组30例(HP+DM组);30名正常健康者作为对照组(NC组),3组患者进行动态血压、血空腹血糖(FBG)、空腹胰岛素(FINS)、血脂分析包括血甘油三酯(TG)、胆固醇(TC)及高密度脂蛋白(HDL)等的检测。结果三组比较FBG、FINS、收缩压(SBP)、TG、TC和IAI有显著性差异,相关分析显示SBP、TG、TC均与IAI成明显负相关。结论高血压与IR密切相关,IR可导致脂质代谢紊乱,血脂异常又参与IR的发生与进展。Objective To explore the relationship between insulin resistance and serum lipids metabolism in elderly hyper- tensive patients. Methods Sixty elderly hypertensive patients were divided into simply hypertension group (HP group, 30 eases) and hypertension combined with type 2 diabetes mdlitns group (HP+ DM group, 30 cases). 30 healthy people were served as the normal control group (NC group). The levels of Ambulatory blood pressure monitoring (ABPM), fasting blood glucose (FBG), fasting insulin (FINS), triglyeefide (TG), cholesterol (TC), high density lipoprotein cholesterol ( HDL- C) were measured in these three groups. Results The levels of FBG, FINS, SBP, TG, TC in liP and HP + DM groups were significantly higher than those in NC group. Insulin sensitivity index (IAI) was lower in liP and liP + DM group than that in NC group. SBP, TG, TC were negatively associated with IAI. Conclusions It was found that statistically significant correlation existed between blood pressure and IR. IR could result in serum lipids metabohe disorder. The latter took part in the development of IR.
