超细型纤维支气管镜在大容量全肺灌洗术中的应用价值

摘　　要：目的评价纤维支气管镜在大容量全肺灌洗术(WLL)中的应用意义。方法对尘肺患者进行489例次WLL,经临床判定两肺分隔满意后,再经纤维支气管镜检证实双腔管对位情况。结果全部病例临床判定分隔满意后,经纤维支气管镜检证实仍有46.6%对位不满意,偏深19.2%,偏浅27.2%。结论WLL术中应用纤维支气管镜检查,提高了双腔管对位的准确性及两肺分隔的满意程度,提高了WLL的安全性,对肺灌洗工作具有重要的指导意义。[Objective] To evaluate the significance of fibrobronchoscope in the process of whole-lung lavage （WLL）. [Methods] Use the fibrobronchoscope to prove the situation of double-lumen tube after verified by the ways of clinical determination in 489 WLL. [Results] There was 46.6% mal-position verified by fibrobronchoscope after the determination by the ways of clinical methods, 19.2% was deeper, 27.2% was superficial. [Conclusion] Fibrobronchoscope used during WLL can increase the accurate position of double-lumen tube and the satisfaction of the separation of two lungs. Fibrobronchoscope plays an important role in the process of WLL.
