冠心病合并糖尿病人群血清脂联素水平研究

摘　　要：目的探讨冠心病合并糖尿病人群血清脂联素、高敏C反应蛋白(hs-CRP)、肿瘤坏死因子-α(TNF-α)、总胆固醇(TC)、甘油三酯(TG)、高密度脂蛋白胆固醇(HDL-C)、空腹血糖(FPG)、空腹胰岛素(FINS)水平并分析其相互之间的关系。方法以172例临床确诊冠心病(经冠状动脉造影证实)同时合并糖尿病的患者(B组)为研究对象,67例健康查体者作为对照组(A组),对上述指标进行检测。采用Gensin i积分系统对冠状动脉狭窄程度进行定量评定。结果(1)糖尿病合并冠心病人群血清脂联素水平(6.85±2.71mg/L)明显低于对照组(13.64±3.24mg/L,P<0.01)。(2)血清脂联素水平和hs-CRP、TNF-α、TG、胰岛素抵抗指数(HOMA-IR)呈负相关,与HDL-C呈正相关。(3)血清脂联素水平与冠状动脉的狭窄程度呈负相关。结论低脂联素水平是冠状动脉粥样硬化发生及病变程度的重要危险因子;2型糖尿病患者血清脂联素水平的明显下降可作为冠心病的危险因素。Objective： To measure the serum concentration of adiponectin, high sensitive C protein （hs - CRP ） , tumor necrosis factor - α（ TNF -α）, total cholesterol （ TC ）, triglyceride （ TG ）, high density lipoprotein cholesterol （ HDL - C ）, fasting plasma glucose （ FPG）, fasting insulin（FINS） in coronary heart disease patients with type 2 diabetes mellitus, and analyse the relationships between them. Methods： The parameters of adiponectin, hs- CRP, TNF- α, TC,TG, HDL- C, FPG, FINS were measured in 172 coronary heart disease patients（ conformed by coronary angiograthy） with type 2 diabetes mellitus （ B group ） and healthy subjects （ A group ）. Results： The serum concentration of adiponectin （ 6.85 ± 2.71 mg/L ） in B group is significantly lower than that of A group （ 6.85 ± 2.71 mg/L, P 〈 0. 01 ）. （ 2 ） The serum concentration of adiponectin is negatively related to hs - CRP, TNF - α,TG, insulin resistance index, and positively related to HDL - C. （3） The serum concentration of adiponectin is negatively related to extent of coronary artery stenosis. Conclusion： The low level of serum adiponectin is related to the advancing coronary artherosclerosis ,The desease of adiponectin serum concentration in Type 2 diabetes mellitus patients is a dependent risk factor for coronary heart disease.
