摘 要:目的探讨我院2004年9月至2006年8月产AmpC酶革兰阴性菌AmpC酶的分离情况及三维实验筛选AmpC酶的效果。方法用VITEK2系统对临床分离茵株进行再鉴定细菌,用三维实验检测AmpC酶。结果在所检测的572株革兰阴性菌中,三维试验阳性菌株共204株,总检出率为35.7%(204/572),其中单产AmpC酶菌株148株,检出率为25.9%(148/572),同时产AmpC酶和ESBLs菌株共有56株,检出率为9.8%(56/572)。572株革兰阴性菌从药敏表型筛选结果分析,表现为高产AmpC酶的菌株共173株,其中148株三维试验阳性,符合率为85.5%。结论AmpC酶在我院分离率较高。三维试验虽烦琐,但更准确,表型测定仍不失为良好的临床实验室常规AmpC酶的筛选方法。Objective To investigate the distribution of AmpC-produeing gram-negative bacilli isolated in our hospital between September 2004 and August 2006. Methods The clinical isolates were rerdentitied by VITEK2, and three dimensional test was used to detect AmpC enzyme. Results Ot 572 clinical isolates identified, 204 were AmpC positive by three-dimensional tests (35.7% including 148 (25.9%) of single AmpC beta-laetamase producers and 56 (9.8%) producing both AmpC beta-laetamase and ESBLs. Phenotype screening test showed that 173 out of 572 isolates were highly AmpC beta-laetamases producing strains, of which, 148 tested positive by three-dimensional test with the coincidence rate as 85.5%. Conclusion High isolation rate of AmpC-produeing gram-negative bacilli was shown in our hospital. Despite its complicated use, three-dimensional test appears reliable to detect AmpC enzyme. Phenotype screening test remains useful as a routine method for AmpC enzyme detection in laboratory practices.
