T细胞、角蛋白10、Ki-67在进展期银屑病皮损和皮损边缘皮肤的表达和意义

摘　　要：目的:探讨T细胞、增殖标记(Ki-67)、角蛋白10阳性细胞在进展期银屑病皮损和皮损边缘皮肤的水平。方法:采用免疫组化法检测19例寻常型银屑病进展期皮损中央、皮损边缘、皮损边缘正常皮肤和远离皮损5cm正常皮肤的T细胞亚群、Ki-67和K10+的表达。结果:皮损边缘CD4+、CD25+T细胞数目明显增多,表皮增殖标记(Ki-67)明显增加,而K10+呈低表达。结论:CD4+、CD25+T细胞在银屑病发病的早期起着一定作用,Ki-67表达水平反映了角质形成细胞的增殖状态,K10+在银屑病皮损的低表达意味着角化过程普遍受损。Objective： To measure the expression of T- cell subsets, markers for epidermal proliferation （Ki- 67） and epidermal differentiation keratin 10 in progressive psoriatic lesion and the edges of the lesion. Methods： Nineteen patients were selected. Specimens taken from central psoriatic lesion, the edge of the lesion, normal skin close to the lesion and the normal skin 5 cm away from the lesion. The levels of expression of T - cell subsets, keratin 10 and Ki - 67 were measured by immunohistochemical staining methods. The skin from normal people was used as control. Results： The number of both CD4^＋and CD25^＋ T lympbocytes increased significantly in the inner margin of the lesion. Compared with control group, Ki - 67 levels in central area of the lesion, inner edge of the lesion and normal skin close to the lesion were over- expressed, while the K10^＋ expressions in these sites were decreased. There was no significant difference in the expression of K10^＋ and Ki-67 between the skin 5 cm away from the lesion and control group. Conelusion： CD4^＋ and CD25^＋ T cells may play a role in the early phase of the psoriatic process. The expression level of Ki - 67 may reflect the over - proliferation of keratinocytes in psoriasis. The low expression of K10^＋ in psoriatic patients suggests that the process of keratinization is generally impaired.
