摘  要:目的探讨颈动脉内膜切除术后早期再狭窄中ICAM-1、MMP-2的表达及其变化规律,为进一步了解颈动脉内膜切除术后早期再狭窄的发生机制。方法以新西兰兔颈动脉粥样硬化性狭窄动物模型为基础,采用免疫组织化学的方法检测颈动脉内膜切除术后不同时间点新内膜中ICAM-1、MMP-2基因的表达。结果对照组中动脉壁中ICAM-1、MMP-2基因呈无表达,ICAM-1在术后4h中膜平滑肌即可见表达,术后3d在新内膜开始表达,术后7d ICAM-1在新内膜的表达达到高峰,随后逐渐下降。MMP-2于术后1d在中膜开始表达,7d在新内膜达到高峰,随后逐渐下降。ICAM-1与MMP-2的表达呈正相关。结论颈动脉内膜切除术后早期再狭窄新内膜ICAM-1、MMP-2表达增高,表明ICAM-1、MMP-2对血管平滑肌细胞的增殖和迁移,介导的局部血管重建和再塑,对再狭窄的形成有重要的作用。Objective To explore the expression and changes of intercellular adhesion molecule-1 ( ICAM-1 ), matrix metalloproteinase-2 ( MMP-2 ) in early restenosis after carotid endarterectomy ( CEA ), analyze of the machanism of early restenosis after CEA. Methods Using New Zealand Rabbits, carotid atherosclerotic stenosis (CASS)model were created. The expression of ICAM-1 and MMP-2 of neointima after CEA were detected by immunohistochemical technique. Results The expression of ICAM-1, MMP-2 were not detected in artery in control group. The expression of ICAM-1 after CEA were seen in vascular smooth muscle cell (VSMC) in media at 4 hours, and seen in neointima at 3 days, and reached maxium at 7 days and gradually descent following; the expression of MMP-2 was seen in media at 1 day, and at the peak in neointima at 7 days and gradually descent following. The expression of ICAM-1 was correlated with MMP-2. Conclusion The increased expression of ICAM-1 and MMP-2 play an important role in VAMC proliferation, reconstruction and remolding of artery and lead to restenosis.
