摘  要:目的探讨检测银屑病患者甲襞微循环改变的临床意义。方法选用国产麦赛3-999Ⅱ型微循环显微镜在室温(18—25℃)下观察50例银屑病患者的左手无名指甲襞微循环,同时与20例健康人作对照。检测管袢形态、流态、袢周状态等指标。结果管袢形态:寻常型银屑病(0.98±0.38)分,红皮病型银屑病3.64分,脓疱型银屑病3.95分;对照组(0.95±0.32)分。流态:寻常型银屑病(1.21±0.38)分,红皮痛型银屑痛0.81分,脓疱型银屑病0.78分;对照纽(1.01±0.36)分。袢周状态:寻常型银屑病(0.96±0.41)分,红皮病型银屑病4.21分,脓疱型银屑病4.09分;对照组(0.78±0.46)分。对照组与寻常型银屑病组比较差异有极显著性意义(P〈0.005)。结论银屑病的甲襞微循环改变以管袢畸形、袢周渗出和出血改变为主,且微循环障碍的程度与病情严重程度成正相关。Objective To study the changes of nail fold microcirculation in psoriatic patients. Methods Fifty cases of psoriasis, including 37 cases of psoriasis vulgaris, 9 cases of psoriasis erythrodermia , 4 cases of psoriasis pustulosi, were classi- fied into therapy group and other 20 healthy subjects served as control. Using MAISAI3-999 Ⅱ microscope connected with a computer, we detected the loop shape, flow state, and periloop state. Results The loop shape was ( 0. 98 ± 0. 38 ) in cases of psoriasis vulgaris, 3.64 in psoriasis erythrodermia, 3.95 in psoriasis pustulosis, and ( 0.95 ± 0. 32 ) in controls. Flow state was ( 1.21±0. 38 ) in psoriasis vulgaris, 0. 81 in psoriasis erythrodermia, 0. 78 in psoriasis pustulosis, and( 1.01±0. 36) in controls. Periloop state was ( 0. 96 ± 0.41 ) in psoriasis vulgaris, 4. 21 in psoriasis erythrodermia, 4. 09 in psoriasis pustulosis, and ( 0. 78 ± 0. 46 ) in controls. There was significant difference between psoriasis vulgaris and controls ( P 〈 0. 005 ). Conclusion There are obvious changes in nail fold microcirculation in patients with psoriasis and the extent of microcirculation disturbance is positively correlated with the severity of pathogenetic conditions.
