芪参降脂饮治疗颈动脉粥样硬化的临床研究

摘　　要：[目的]探讨芪参降脂饮对颈动脉粥样硬化(atherosclerosis,AS)患者血脂、胰岛素敏感性及颈动脉内膜-中膜厚度(intima-media thickness,IMT)、斑块面积变化的影响。[方法]选择经彩色多普勒超声确诊有颈AS证据的患者作为观察对象,给予芪参降脂饮口服,并设舒降之(辛伐他汀)组进行对照。所有观察对象于治疗前后同步检测血脂、FPG、FINS,计算胰岛素敏感指数(insulin sensitivity index,ISI),检查颈动脉IMT、斑块面积。[结果]颈AS患者应用芪参降脂饮治疗后,两组治疗后血脂较治疗前改善,ISI(-1.69±0.23、-1.67±0.69)较治疗前(-1.83±0.33、-1.84±0.26)升高,差异均有显著性意义(P<0.05)。两组间治疗后比较差异无显著性意义(P>0.05);芪参降脂饮治疗组颈动脉IMT(0.86±0.17)mm、粥样斑块面积(11.47±9.17)mm2较治疗前(1.27±0.21)mm、(16.42±10.69)mm2明显减小(P<0.01),舒降之对照组无上述作用。[结论]芪参降脂饮治疗颈动脉粥样硬化患者,可通过降低血脂水平,改善胰岛素抵抗(insulin resistance,IR),减轻AS病变。[Objective] To study the effects of Qishenjiangzhiyin（QSJZY） on Serum lipids,insulin sensitivity（IS） and intimamedia thickness（IMT） or atherosclerotic clots with carotid atherosclerosis patients. [ Methods] All patients were examined with color ultrasound and diagnosed as carotid atherosclerosis. Those who dosed with QSJZY were served as treatment group and who dosed with simvastatin were served as control group, FPG,FINS were measured and insulin sensitivity index （ISI）were calculated , IMT or area of atherusclerutic clots were examined before and after treatment. [ Results ] In both treatment group and, compared with before experiment, TC,LDL - C,FPG and FINS were significantly lower while HDL - C and ISI（ - 1.69 ±0.23, - 1.67 ±0.69） were significantly higher after treatment than before treatment（ - 1.83±0.33, - 1.84 ±0.26） （P 〈0.05）. No significant difference exists in both groups. IMT （0. 86±0.17） mm or area of atherosclerotic clots （11.47 ± 9. 17 ） mm^2 were decreased in treatment group contrast to those （1.27± 0. 21 ） mm, （ 16.42 ± 10.69） mm^2 before QSJZY treatment, whereas no significant changes in control group. [ Conclusion ] QSJZY can cure carotid atherusclerusis patients by lowering serum lipids and reducing insulin resistance（IR）.
