摘 要:目的探讨腰椎后路棘突椎板作为骨源椎间植入加椎弓根钉系统固定在下腰椎椎间融合中的可行性和临床效果。方法应用自体棘突椎板作为骨源椎间植入加椎弓根钉系统固定下腰椎98例,其中椎间盘脱出症30例,椎间盘突出症复发11例,腰椎不稳45例,椎管狭窄12例。结果10例失访,88例术后随访12—56个月,腰腿痛症状消失或明显减轻,腰椎间融合好,椎间隙高度无明显减少。结论棘突椎板作为骨源椎间植入加椎弓根钉系统固定在下腰椎的应用具有操作简单、安全、可靠及融合效果好的优点。Objective To explore the feasibility and clinical effects of spinal processes and laminae as materials for interbody fusion and pedicle screw fixation in the lower lumbar spine. Methods The technique of spinal processes and laminae as material interbody fusion and pedicle screw fixation was performed in the lower lumbar spine in 98 patients, including 30 cases of lumbar disc extrusion, 11 cases of recurrent of lumbar disc herniation ,45 cases of instability of lower lumbar spine, 12 cases of spinal canal stenosis. Results 10 cases were lost. 88 cases were followed up for 12 - 56 months. The pain in the lumbar spine and lower limb disappeared or alleviated obviously. Lumbar interbody fusion was good and the height of lumbar intervertebral space was not lost significantly. Conclusions Spinal processes and laminae as materials for interbody fusion with pedicle screw fixation in the lower lumbar spine has advantages of simple operation and salty, and credibility, and good fusion.
