降浊止呕颗粒对延缓慢性肾衰竭进展的临床研究

摘　　要：目的:观察降浊止呕颗粒治疗慢性肾功能衰竭(CRF)脾虚湿浊证的临床疗效,试探讨其延缓慢性肾衰竭进展的作用机制。方法:CRF患者90例,随机分为治疗组和对照组。治疗组给予降浊止呕颗粒,对照组给予尿毒清颗粒,疗程4周。观察治疗前后尿素氮(BUN)、血肌酐(Scr)、肌酐清除率(Ccr)、血红蛋白(Hb)、红细胞计数(RBC)、血钙(Ca)、血磷(P)等指标及临床症状的变化,做出疗效评价。结果:①治疗组与对照组总有效率比较,无显著性差异(P>0.05),显效率比较有统计学意义(P<0.05)②中医证候疗效比较有显著性差异(P<0.05),治疗组优于对照组。③两组治疗后均能显著改善临床症状及降低症状积分,治疗组对临床症状的改善及积分的下降程度优于对照组(P<0.01)。④两组患者血肌酐(SCR)、尿素氮(BUN)均有不同程度的下降,治疗组SCR、BUN下降程度较对照组有显著差异(P<0.05或P<0.01)。⑤两组患者治疗后RBC、HB均无改善,(P>0.05)。⑥治疗后,两组患者自身比较,治疗后血钙略有升高、血磷降低(P<0.01),但两组组间比较无统计学意义(P>0.05)。结论:降浊止呕颗粒通过健脾益气、降浊排毒的作用,对CRF辨证属脾虚湿浊证为主者,起到显著改善临床症状,改善肾功能,从而能有效的延缓慢性肾功能衰竭的进展。To observe the clinical curative effect of Granules for Reducing the Turbid and Preventing the Vomiting on patients with the chronic renal failure （CRF） cauaed by stagnation of wetness in the body with deficiency of spleen. The article is to probe the functional mechanism of delaying the progress of CRF. Methods： 90 patients with the chronic renal failure were divided into two groups randomly： the treat group 60 cases and the control group 30 cases. The treat group was given Granules for Reducing the Turbid and Preventing the Vomiting, and the control group was given Niaoduqing Granules. The course of treatment for each group was 4 weeks. Observe the change of Blood urea nitrogen （BUN）, Serumcreatinine （SCr） and clinical symptoms in before and after therapy. At last observe the therapeutic effect. Results： 1. After a period of treatment, Granules for Reducing the Turbid and Preventing the Vomiting can improve conditions more evidently than Niaoduqing Granules （ P 〈 0. 01）. 2. For patient in the treated group, Granules for Reducing the Turbid and Preventing the Vomiting can significantly decrease the level of SCr and BUN. When compared with the control group, it appeared difference （P 〈0.05orP 〈0.01）. Conclusion： Granules for Reducing the Turbid and Preventing the Vomiting can effectively improve conditions of patient with the chronic renal failure caused by stagnation of wetness in the body with deficiency of spleen syndrome, so it can effectively delay the progress of CRF.
