摘  要:目的探讨乳腺浸润性非特殊型癌钼靶X线表现与组织学分级之间的关系。方法回顾性研究2004年6月—2005年6月间100例经手术病理证实为乳腺浸润性非特殊型癌的影像学表现,以及影像学表现与肿瘤的组织学分级相关。结果20%的组织学分级为Ⅰ级的肿瘤及72%的组织学分级为Ⅲ级肿瘤X线表现肿块,68%的Ⅰ级肿瘤及8%的Ⅲ级肿瘤在钼靶X线表现边缘毛刺征,12%的Ⅰ级肿瘤及21%的Ⅲ级肿瘤X线表现边缘不清,8%的Ⅰ级肿瘤及23%的Ⅲ级肿瘤X线表现恶性细小钙化。结论乳腺Ⅰ级肿瘤的钼靶X线表现多具有典型的恶性肿块征象如边缘毛刺征,而Ⅲ级肿瘤往往边缘清楚。因此,Ⅲ级乳腺浸润性非特殊型癌可能与乳腺良性肿瘤的X线表现类似。Objective To investigate the relationship between mammography findings and histological grade in patients with non-special type invasive breast carcinoma. Methods A retrospective study was performed on the imaging findings in 100 patients diagnosed with non-special type invasive breast carcinoma between June 2004 and June 2005. Analysis of rank correlation was applied in the relation of imaging findings with the histological grades of tumor. Results A mass imaging in mammogram was found in 20% of low-grade tumors and in 72% of high-grade tumors. A speculated margin in mammogram was documented in 68% of low-grade tumors compared with only 8% of high-grade tumors. 12% of low-grade and 21% of high-grade tumors had a poorly defined margin in mammography. Malignant-type microcalcification was seen in mammogram in 8% of low-grade tumors and in 23% of high-grade tumors. Conclusion The classical appearance of a malignant breast cancer was a speculated mass in mammogram and was a more typical imaging of low-grade tumor. In comparison, high-grade tumors were more likely to demonstrate well-defined margin imaging. Therefore, high-grade non-special type invasive carcinoma may display imaging features as similar as the benign breast cancer.
