肾病合剂配合激素治疗小儿单纯型肾病综合征68例临床研究

摘　　要：目的探讨肾病合剂对激素治疗小儿单纯型肾病综合征的干预作用。方法以肾病合剂配合强的松治疗儿童单纯型肾病综合征68例(治疗组),并随机设立强的松组(对照组)33例,观察疗效及血皮质醇、免疫指标的变化。结果治疗组水肿消退时间、尿蛋白转阴时间和治疗6个月及1年时持续缓解率、复发率均优于对照组(P<0.01或P<0.05)。治疗后血清皮质醇、血清免疫球蛋白G、血清免疫球蛋白A浓度与CD3、CD4及CD4/CD8均明显升高,与正常组比较差异无显著性意义(P>0.05)。结论肾病合剂能明显提高激素治疗小儿肾病的敏感性,促进临床症状的改善并减少复发,其作用机制与对机体免疫的调节以及促进肾上腺皮质功能恢复等多方面的干预作用有关。Objective To study the intervention of Shenbing Mixture combined with hormone for simple nephrotic syndrome in children. Methods Control group of 33 cases was treated by Prednisone,while Shenbing Mixture was added in treatment group of 68 cases. The therapeutic effect,hydrocortisone and immunologic index were observed. Results The treatment group was superior to control group in regression time of edema,negative conversion time of urine protein and persistent remission and relapse rates after 6- month and one-year treatment （P〈0.05 or P〈0.01）. The level of hydrocortisone,immunoglobulin G,immunoglobulin A, CD3, CD4 and CD3/CD4 in the treatment grouup increased obviously with no significant differences when compared with those of normal group（P 〉0.05）. Conclusion Shenbing Mixture can obviously improve sensitivity of hormone,relieve symptoms and reduce relapse for pediatric nephrosis,which is relevant to regulating immunity and recovering the function of adrenal cortex.
