妊娠期糖尿病患者胎儿脐血血清中SHBG水平的变化及意义

摘　　要：目的检测妊娠期糖尿病(GDM)患者其新生儿脐血血清中性激素结合球蛋白(SHBG)、血糖、胰岛素及IR的水平。探讨其在新生儿脐血中的变化规律及对新生儿生长发育的影响。方法采用酶联免疫吸附双抗体夹心法(ELISA)测定22例GDM患者和21例正常妊娠妇女新生儿血清中SHBG的浓度,用放免电化学发光法检测血清中的胰岛素(INS)浓度。结果GDM组实验组脐血中SHBG浓度、血糖水平均明显低于正常对照组,差异显著(P<0.01);空腹胰岛素浓度明显高于正常对照组,差异显著(P<0.01),IR在两组中无明显差异(P>0.05)。SHBG与体重及脐血FIns浓度均呈负相关。结论SHBG与肥胖关系非常密切,它可能是早期肥胖及DM发生、发展的高危因素,也可能成为预测新生儿发展为肥胖的一个重要指标。Objective： To investigate the level of sex hormone binding globulin, FBG, Fins and IR in cord blood serum with Gestational diabetes mellitus, in order to find their effects to newborns. Methods ： Enzyme linked immunosorbent assay （ELISA） was used to determine serum SHBG levels in 22 newborns with Gestational diabetes mellitus and 21 with normal pregnant women. Results： The SHBG and FBG levels of cord blood in GDM groups were significantly lower than that in the control group （ P 〈0. 01 ） ; and the levels of Fins were significantly higher than those in the control group （ P 〈0.01 ）. IR in two groups were no remarkable difference. There was a negative correlation between SHBG and body weight of newborns, also between SHBG and Fins. Conclusion： There is a correlation between SHBG and fat. It may be a high risk of early stage of fat and DR. SHBG may also be a index to judge the newborn to be fat in the future.
