骨髓增生异常综合征患者红细胞免疫黏附功能的研究

摘　　要：目的 探讨骨髓增生异常综合征（MDS）患者红细胞免疫黏附功能的变化及其与疗效及预后的关系。方法 应用红细胞酵母菌花环试验测定38例健康献血者（对照组）和35例初次确诊为MDS患者（患者组）缓解前后红细胞膜C3b受体花环率（RBC-C3bRR）及红细胞膜免疫复合物花环率（RBC-ICR）。结果 对照组RBC-C3bRR及RBC-ICR分别为（18．65±2．58）％和（7．45±2．16）％；患者组缓解前后RBC-C3bRR分别为（9．87±2．43）％和（14．65±3．22）％，而缓解前后RBC-ICR分别为（15．48±3．25）％和（9．36±2．56）％。患者组缓解前后RBC-C3hRR及RBC-ICR与对照组比较差异均有统计学意义（P均〈0．01）；患者组缓解前后RBC-C，bRR及RBC-ICR比较差异有统计学意义（P〈0．01）；缓解后RBC-C3bRR明显升高，但仍较对照组低，缓解后RBC-ICR明显降低，但仍较对照组高。结论 MDS患者红细胞免疫黏附功能下降，测定红细胞免疫黏附功能（RBC-C3bRR、RBC-ICR）可作为判定MDS患者疗效及预后的指标之一。Objective To study the change of immune adhesive function of red blood cells （RBC） in myelodysplastic syndrome （MDS） patients. Methods Used the RBC yeast - rosette test, 38 normal people （control group） and 35 MDS patients（patient group） before and after remission were detected about RBC membrane C3b receptors rosette rate （RBC -C3bRR） and RBC membrane circulation immune complexes rosette rate （RBC -ICR）. Results The RBC -C3bRR and RBC -ICR in control group were （18.65±2.58）% and （7.45 ±2.16）%. The RBC - C3bRR in patient group before and after remission were （9.87 ± 2.43 ） % and （ 14.65 ± 3.22 ） %, the RBC - ICR were （ 15.48 ± 3.25 ） % and （9. 36 ± 2.56 ） %. Before and after remission the RBC - C3b RR and RBC - ICR in patient group were difference as compared with those in control group （P〈0.01）. In patient group, before remission RBC - C3bRR and RBC - ICR were difference as compared with those after remission （ P 〈 0.01 ）, the level of RBC- C3b RR after remission was higher than that before remission, but was lower than that in control group,the level of RBC -ICR after remission was lower than that before remission,but was higher than that in control group. Conclusion The immune adhesive function of RBC in MDS patients is lower,the test of the RBC immune adhesive function （ RBC - C3bRR and RBC - ICR） can be taken for a referential index to judge the clinical effect and evaluate prognosis of MDS patients.
