糖尿病肾病不同药物治疗方案的疗效观察及经济学评价

摘　　要：目的:评价丁咯地尔与前列地尔治疗糖尿病肾病的临床疗效及经济学意义。方法:回顾性分析糖尿病肾病住院患者病例,统计分析两组治疗前后血尿素氮、肌酐、尿酸和尿微量白蛋白检验值,并计算两组相应的经济学指标。结果:治疗后两组血肌酐、尿微量白蛋白和血尿酸有统计学差异,前列地尔组血肌酐、尿微量白蛋白临床疗效优于丁咯地尔组,丁咯地尔组血尿酸下降较前列地尔组明显,治疗后两组血尿素氮无统计学差异。治疗方案的例均总成本丁咯地尔组和前列地尔组分别为12 808.82元和14 251.29元,无统计学差异。丁咯地尔组和前列地尔组血肌酐、血尿酸、尿微量白蛋白3项检验指标的成本-效果值分别为:1 730.92,98.30,2 891.38和914.13,117.83,2 482.80。结论:2种治疗方案均可改善糖尿病肾病的临床指标,但对不同指标作用有所差异。2种治疗方案的成本不存在统计学差异,而2种治疗方案的成本-效果比值明显不同。Objective ： To compare the efficacy and economic cost of bufiomedil to alprostadil for diabetes nephrosis. Methods： A retrospective analysis of in-patients with diabetic nephrosis who were treated with buflomedil versus alprostadil was conducted. The efficacy was statistically assessed based on the status of levels of blood urea nitrogen, creatinine, uric acid and urinary micro-albumin before and after the treatment. Costs of both regimens were compared. Results： The response to improve serum creatinine and urinary micro-albumin by alprostadil was superior to the one by buflomedil. The buflomedil-treated patients experienced more significant reduction of serum uric acid than the patients treated with alprostadil. No statistical difference of BUN by both regimens was found. The total cost of buflomedil versus alprostadil was ￥ 12 808.82 and ￥ 14 251.29, respectively （ P 〉 0.05 ）. The ratio of cost and effect （C/E） of buflomedil and alprostatdil for the improvement of creatinine, uric acid and micro-albumin was 1 730.92, 98.30, 2 891.38 versus 914.13, 117.83, 2 482.80, respectively. Conclusion： No significant cost difference of alphostadil from deflomedil was found. The C/E difference of these two regimens was distinct.
