摘 要:目的探讨高渗糖对糖尿病肾病血液透析患者透析相关性低血压的预防和治疗效果。方法21例发生透析相关性低血压的糖尿病肾病患者,每例患者观察2个月(24次血透),前1个月透析过程禁止使用50%葡萄糖(无糖透析)作为自身对照,后1个月透析过程使用50%的葡萄糖(输糖透析)作为观察组,记录透析过程血压变化,观察低血压及其他并发症的发生情况。结果两组血液透析前的收缩压和舒张压比较无统计学意义(P>0.05);21例患者输糖透析和无糖透析中低血压和并发症的发生率比较,差异均具有统计学意义(P<0.05);透析过程最低血压与透析前血压的差值比较差异有统计学意义(P<0.01)。结论透析过程输注50%葡萄糖有助于预防糖尿病肾病患者透析相关性低血压及其他并发症的发生,改善患者的生活质量。Objective To investigate the therapeutic effect of hyperosmotie glucose in preventing the incessant dialysis hypotension (IDH) and preventing and treating complications in diabetic nephropathy patients. Methods 21 cases of diaberie nephropathy patients, who received regular hemodialysis, were observed for two months (24 times hemodialysis). As self-control group, 50% hyperosmotie glucose was forbidden in the first month and used in the next month. The change of blood pressure, the hypotension and other complications during dialysis were recorded and analyzed. Results There was no significant difference between two groups in SBP and DBP before hemodialysis (P〉0.05), but there was significant difference in the incidence of hypotension and other complications (P〈0.05) and difference between SBP and DBP (P〈0.01). Conclusion It is helpful to prevent hypotension and other complications and improve their quality of life by transfusion 50% glucose into diabetic nephropathy patients during hemodialysis.
