生物梅里埃第4代HIV初筛试剂假阳性率增高问题的探讨

摘　　要：〔目的〕寻求更为敏感和特异的用于出入境人群的HIV初筛检测方法或检测流程,及时准确出具HIV抗体检测报告。〔方法〕从本中心HIV确认实验室的送检标本中,选取经免疫印迹法(WB)确认为HIV抗体阳性标本30份,和经WB法确认为阴性及不确定标本30份,分别用生物梅里埃和丽珠试剂同时做ELISA法筛查,比较其阳性率。分析比较近3年来由于生物梅里埃试剂换代,确认实验室送检样品阳性检出率情况。〔结果〕生物梅里埃试剂在用于HIV初筛检测中假阳性率高于丽珠试剂。随着梅里埃试剂由第3代更换为第4代,其检测假阳性率提高显著。〔结论〕用于出入境人群的HIV初筛检测试剂应选择敏感性高、特异性高的试剂;在保证结果准确的情况下,可以应用《全国艾滋病检测技术规范(2004年版)》第2章HIV抗体检测中的替代策略3,以便快速准确地出具HIV检测报告。Objective To find more sensitive and specific methods and inspect procedures of the test of HIV screen among the entry-exit personnels. Methods Among all of samples tested in HIV confirm lab,in which, 30 HIV-positive samples, 30 HIV-negative and HIV-uncertain samples were chosen, meanwhile they were confirmed by WB.All the samples were detected with different reagents of Bio-Merieux and Lizhu respectively in comparison of the positive rates.The positive rates were analyzed because of the Bio-Merieux reagent update. Results The false positive rate of the reagents from Bio-Merieux was higher than that from Lizhu. The false positive rate was increased significantly after reagents update. Conclusion More sensitive and specific reagents should be selected for the test of HIV screen among the entry-exit personnels.According to the National Guideline for Detection of HIV/AIDS （2004）,the second chaper of replacing policy Ⅲ could be used for the entry-exit personnels more correctly and rapidly.
