摘  要:目的探讨妊娠糖尿病(GDM)患者胰岛素抵抗(IR)与脂联素(ADI)、瘦素(Leptin)的关系。方法测定40例GDM患者(GDM组)及34例正常人(对照组)空腹血糖(FPG)、胰岛素(FINS)、ADI、Leptin、总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白(LDL)、高密度脂蛋白(HDL),以及餐后2h血糖(2hPG)、胰岛素(2hINS),计算胰岛素抵抗指数(IRI)。结果两组ADI、Leptin、IRI比较有统计学差异(P<0.05)。Pearson相关分析显示,GDM组IRI与年龄、孕周、体质量指数(BMI)、TG、TC、LDL、FPG、FINS、2hINS、Leptin呈正相关(P<0.05,<0.01),与ADI呈负相关(P<0.01)。多元线性逐步回归分析结果显示,BMI、ADI、Leptin是影响GDM患者IR的独立危险因素。结论GDM患者存在明显IR,ADI、Leptin与GDM患者IR有关。[Objective] To investigate the relation between insulin resistance(IR) and adiponection, leptin in patients with gestational diabetes mellitus (GDM). [Methods] Fasting ptasma glucose (FPG), fasting serum insulin (FINS); adiponectlon, leptin, total cholesterol (TC),triglyceride (TG).low density lipoprotein (LDL), high density lipoprotem(HDL) as well as 2 h serum plasma glucose (2 hPG), 2 h serum insulin (2 hINS) were determined in40 patients width GDM and 34 control subjects. The insulin resistance index (IRI) was calculated at the same time. [Results] ADI,leptin and IRI in two groups were obviously different (P〈0. 05). Pearson correlation analysis indicated that IRI in GDM group was positively correlated with age, pregnant weeks, BMI, TG, TC,LDL, FPG, FINS, 2 hINS and leptin (P 〈 0. 05,〈 0.01), but negatively correlated to adiponection (P 〈 0.01). A multiple linear stepwise regression analysis indicated that BMI,leptin and adiponection were independent factors that affected IRI. [Conclusion] IR exists in the CDM patients,ADI and leptin have positive relation with IR in GDM patients.
