摘 要:目的比较两种不同方法服用非洛地平对24h动态血压的影响,从而寻求一种较好的服药方法,以获得良好的降压效应。方法120例1-2级高血压患者随机分成A组和B组,A组每晨6时口服非洛地平10mg。B组每晨6时、下午4时分别口服非洛地平5mg,比较两组患者4周服药后平均血压下降值、血压负荷、平滑指数。评价降压效果。结果服药后两组患者的24h平均收缩压、平均舒张压、白昼平均收缩压、平均舒张压均较服药前有明显下降,但两组间比较,差异均无统计学意义(t分别=0.89、1.01、0.55和0.73,P均〉0.05)。B组夜间平均收缩压、平均舒张压、24h收缩压血压负荷、舒张压血压负荷、夜间收缩压血压负荷、舒张压血压负荷低于A组,两组间比较,差异均有统计学意义(t分别=2.11、2.05、3.02、2.95、5.52和8.20,P均〈0.05)。A组收缩压平滑指数为1.12±0.54,舒张压平滑指数为1.02±0.42,B组收缩压平滑指数为1.32±0.62,舒张压平滑指数为1.24±0.46,B组大于A组,两组比较,差异均有统计学意义(t=3,85、2.75,P均〈0.05)。结论两种方法口服非洛地平均能控制1-2级高血压患者24h血压,在平稳降压方面非洛地平5mg早晚两次口服优于非洛地平10mg每晨一次口服。Objective To compare the impact of two different methods of taking feledipine on 24 - hour ambulatory blood pressure, to find a better antihypertensive effect. Methods One hundred twenty patients with grade 1 or 2 hypertension were randomly divided into group A and group B, Group A took 10 mg of feledipine at 6:00 am, while group B take 5rag of felodipine at 6:00 am and 16:00 pm. Four weeks later, the decreased value of the average blood pressure, blood pressure load, the smoothness index, and blood pressure evaluation of the patients between group A and group B were compared. Results There was a marked decline in the average systolic and diastolic blood pressure of 24 hours and average systolic and diastolic blood pressure in daytime . There was no significant difference between the two groups (t = 0.89, 1.01, 0.55, 0.73, P 〉 0.05) . The average systolic and diastolic blood pressure in night time and the 24 systolic blood pressure load and diastolic blood pressure load of group B were lower than that of group A. The systolic blood pressure load and diastolic blood pressure load in night of group B were lower than that of group A, the difference between the two groups was statistically significant( t = 2.11,2.05, 3.02, 2.95,5.52,8.20, P 〈 0.05) . The systolic smoothness index was 1.12 ± 0.54 and the diastolic smoothness index was 1.02 ± 0.42 in group A, while the systolic smoothness index was 1.32 ± 0.62 and the diastolic smoothness index was 1.24 ± 0.46 in group B. The difference between the two groups was statistically significant( t = 3.85,2.75, P 〈 0.05). Conclusions Both the different methods of taking felodipine can control the average 24 - hour blood pressure in patients with grade 1 or 2 hypertension. But taking felodipine 5 rag orally twice is superior to taking felodipine 10 mg every morning in smooth control.
