摘 要:目的总结老年消化道恶性肿瘤患者术后肠内营养的临床护理体会。方法回顾性分析了33例接受各种消化道手术老年患者的临床资料,所有患者均在术中行空肠造瘘,术后接受肠内营养治疗。结果31例患者完成肠内营养治疗,无导管堵塞或脱出,平均输注营养液时间为(10.4±5.9)天,平均术后肛门排气时间为(42.4±15.9)小时,术后平均住院时间为(16.7±17.2)天。16例患者出现腹胀,7例出现腹泻,13例出现糖代谢严重异常。出院前患者营养指标轻度下降,无死亡病例,伤口裂开1例,感染并发症2例,非感染并发症1例(心绞痛)。2例患者因严重腹胀及腹泻中止肠内营养。结论消化道手术后肠内营养的护理应坚持无菌配液、严格管路护理、注意血糖变化和加强心理治疗。Objective To summarize nursing care experience of postoperative enteral nutritional support for elderly patients with malignant gastric cancer. Methods The clinical data of 33 elderly patients who had received gastric operations were retrospectively reviewed. All patients received jejunum stoma during operations, and received enteral nutrition supports after the operations. Results Totally 31 patients completed EN treatment, and two patients stopped EN due to severe abdominal distention and diarrhea. Among these 31 patients, no catheter obstruction or emergence was observed in these patients. The infusion time of EN was 10.4 ± 5.9 days. The anus gas passage time was 42.4 ± 15.9 hours. Adverse events included abdominal distention ( n = 16), and diarrhea (n = 7 ). Abnormal serum glucose was observed in 13 patients. No patient died. One patient experienced wound dehiscence and 2 patients suffered infectious complication. The average post-operation hospital stay was 16.7 ± 17.2 days. Conclusion Strict nursing rules shall be obeyed during nursing care of postoperative enteral nutrition supporting for elderly patients with gastric cancer.
