下肢骨折内固定术后感染更换半环槽式外固定器的护理

摘　　要：目的观察下肢骨折内固定术后感染更换半环槽式外固定器固定的围手术期护理及临床效果。方法对39例下肢骨折内固定术后感染患者更换半环槽式外固定器。术前予心理护理、术前准备,术后做好心理、疼痛、环境、饮食以及外固定器的护理。同时加强患肢的观察、感染的控制以及功能锻炼指导。结果39例患者感染均得到控制,局部切口愈合,术后8~18个月拆除外固定器,下肢功能恢复满意,无畸形及关节僵硬。结论半环槽外固定技术是下肢骨折内固定术后感染较理想的治疗方法,而围手术期护理在感染控制和肢体康复过程中起着重要作用。Objective To observe the perioperative nursing and clinical effect of replacing the internal fixation with external skeletal fixation in treating the postoperative infection for fracture of lower limb. Methods Thirty-nine cases were exchanged with external skeletal fixation in the postoperative infection for fractures of lower limb treated with the internal fixation. The nursing measures included the preoperative and the postoperative mental nursing, pain nursing, comfortable surrounding, suitable food, af- fected limb observing, controlling infection, guiding for functional exercise and the nursing of external skeletal fixation. Results The infections were controlled and the wounds were healed in 39 cases. The external skeletal fixation was dismantled in postoperative 8- 18 months. The limb had good function recovery and had no abnormalities and stiffness of joint. Conclusion The external skeletal fixation is a good approach to treat the postoperative infection for fracture of lower limb treated with the internal fixation. And the perioperative nursing has considerable effect for controlling infection and recovering the function of lower limb.
