吡格列酮对早期糖尿病肾病患者血液流变学及胰岛素抵抗的影响

摘　　要：目的探讨吡格列酮（瑞彤）对早期糖尿病肾病（EDN）患者血液流变学及胰岛素抵抗的影响。方法将74例早期糖尿病肾病患者随机分为观察组和对照组（各37例）。两组均采用饮食控制和糖尿病常规治疗，观察组加瑞彤口服治疗，疗程24周，观察两组治疗前后及组间相关指标的变化。结果观察组血糖、血胰岛素、血脂、尿白蛋白排泄率（UAER）、血液流变学各项指标显著下降，胰岛素抵抗明显改善，与治疗前及对照组相比有显著性差异（P〈0．01）。结论瑞彤能改善EDN患者的血液流变性、改善胰岛素抵抗、减少尿白蛋白的排泄率，保护肾功能。Objective To explore the clinical efficacy of changes of hemorrheology and insulin resistance in patients treatment with Pioglitazone on with early diabetic nephropathy （EDN）.Methods seventy-four patients with EDN were randomly divided into the observation group （n=37） and control group （n=37） . both groups were treated with conventional therapy for diabetes and diet control method .The observation group was given Pioglitazone 15mg once a day and the duration of therapy was 24weeks. The relative indexes between the two groups were observed and compared before and after treatment. Result In the observation group, the blood glucose, insulin ,blood-fat , albumin excretion rate and indexes of hemorrheology were decreased significantly; insulin resistance was improved obviously ;which were significantly different from those before treatment and those of control group （P〈0.01）.Conclusion Pioglitazone can improve the indexes of hemorrheology, insulin resistance, decrease albumin excretion rate, and protect kidney function.
