宁夏农村高血压病人药物治疗依从性及影响因素分析

摘　　要：目的 了解宁夏农村高血压病人药物治疗依从性（CPAT）及影响因素.方法 采用分层整群抽样方法,抽取宁夏永宁、中卫两县四乡14个村、队,由经培训的临床医师对辖区内30～80岁的常住居民（居住该地5年及以上）2276人测量血压,确诊高血压患者580人,以村为单位按研究入选标准随机抽取部分研究对象分别服用北京降压0号（1片/日）、寿比山（2.5 mg/d）.服药一年后对服用寿比山81人、北京降压0号57人及与两组患者同村居住、年龄相差不超过2岁但服用其他降压药69人,进行CPAT调查.结果 207名高血压病人服药依从性佳者为32.7%,单因素Logistic回归分析结果显示服药依从性与年龄（OR=1.044,95%CI1.007～1.082）、服药种类（OR=1.605,95%CI1.089～2.365）、服药次数（OR=1.904,95%CI 1.218～2.974）及服药费用（OR=1.201,95%CI 1.029～1.402）有关,而性别、文化程度、患病年限等与依从性无关;多因素Logistic回归分析结果显示服药依从性与年龄（OR=1.042,95%CI 1.003～1.082）、服药次数（OR=1.932,95%CI 1.203～3.074）有关.结论 宁夏农村高血压病人服药依从性较低,应采取综合有效措施提高依从性.Objective To explore the compliance of patients with antihypertensive therapy （CPAT） and analyze its influence factors in rural areas. Methods TO interview patients in rural areas with special questionnaire. Results The ratio of good CPAT was 32.7%. The Logistic regression analysis results showed： age（ OR = 1. 044,95% CI1.007 - 1.082）,the kinds of taking medicine（ OR = 1.605,95% CI1.089- 2.365）,the number of taking medicine（ OR = 1.904,95% CI1.218 - 2.974） ,the costs for taking medicine（ OR = 1.201,95% CI1. 029- 1.402）were associated with CPAT. There were no association between the sex, education, the years having been ill and CPAT. The multivariate Logistic regression analysis results showed： age （ OR = 1. 042, 95 % CI1.003 - 1.082）, the number of taking medicine （ OR = 1.932,95% CI1.203 - 3.074） were associated with CPAT. Conclusion The rate of good CPAT is low. The synthetic and effective measure of antihypertensive is mainly taken by patients in rural areas, which is helpful to improve the compliance of patients.
