摘  要:目的监测北京部分地区肠道致病菌的组成及耐药状况,为本地区的流行病学研究及临床合理用药提供依据。方法480份粪便标本按常规培养,筛选出致病菌后经生化及血清学进一步鉴定到种、群或血清型,并进行药敏试验。结果肠道病原菌感染,以男性为主,儿童和青年发病为多,6~8月为腹泻发病高峰。病原以致泻性大肠杆菌居首位,其次是志贺菌。本次检测出肠道致病菌对氨苄西林(67.8%)、甲氧苄氨嘧啶(67.8%)等均有严重的抗药性;对哌拉西林(42.5%)、氨苄西林-舒巴坦(33.0%)、呋喃妥因(30.5%)等也均产生了不同程度的抗药性:只有对关洛培南(0%)、磷霉素(1.1%)、头孢塞肟-克拉维酸(3.4%)、头孢他啶(4.6%)、氨曲南(5.2%)均比较敏感。结论北京地区感染性腹泻的病原种类繁多,有性别、年龄、季节的分布特点,耐药性不同,应重视监测。Objective To monitor the distribution and drug resistance of enteric pathogenic bacteria in some areas of Beijing to offer the basis for guiding epidemiologic study and clinical treatment. Methods A total of 480 stool specimens were cultured, and enteric pathogenic bacteria were screened and identified to species, group, and serotype with biochemical and serologic tests. Then, the susceptibility of bacteria to antimicrobial agents was tested. Results Enteric pathogenic bacteria infection was predominantly found in male, children, and youth. The peak incidence of diarrhea was in June and July. Escherichia coli and Shigella were the main pathogenic bacteria of intestinal tract. Seventeen specific antibiotics were selected for determining the susceptibility of bacterial pathogens isolated from feces samples. The resuits showed that the percentage of resistance to antibiotics were: meropenem (0%), fosfomycin (1.1%), cefotaxime - clavulanic acid (3.4%), ceftazidime (4.6%), aztreonam (5.2%), furalan (30.5%), ampicillin-sulbactam (33.0%), piperacillin (42.5%) ampicillin(67.8%), and trimethoprin (67.8%). Condusions There are many species of enteric pathogenic bacteria causing infectious diarrhea in Beijing areas. Their distributions are different in sex, age and season, and the drug resistance rates are different as well, so surveillance is necessary.
