摘  要:目的探讨在控制性超排卵中,晚期卵泡期孕酮水平与体外受精结局的关系。方法2003年4月~2004年12月在该院生殖中心接受体外受精助孕的不孕症患者1408例,其中短方案垂体降调节692人,按照注射HCG日血清孕酮浓度的不同分为3组:血清孕酮≤1ng/mL者462例,血清孕酮1~2ng/mL211例,血清孕酮>2ng/mL9例,长方案垂体降调节716人,血清孕酮≤1ng/mL者588例,血清孕酮1~2ng/mL108例,血清孕酮>2ng/mL20例,回顾性分析体外受精的结局。结果长短方案各组间获卵数,受精率,卵裂率,优胚率无明显差异,随着孕酮水平的升高,着床率和妊娠率逐渐降低,自然流产率升高,但是无统计学差异。结论控制性超排卵中,卵泡晚期孕酮水平对着床率,临床妊娠率,自然流产率无显著影响。[Objective] To evaluate the impact of progesterone level of late follicular phase on outcome of in vitro fertilization in controlled ovarian hyperstimulatlon. [Methods] 1408 cycles of IVF pedormered in our reproduction central from April 2003 to December 2004 were analyzed retrospectively. 692 cycles were used with down-regulation short protocol, and patients were divided into three groups according to serum progesterone level on the day of HCG administration, ≤ 1 ng/mL 462 cycles, 1n-2 ng/mL 211 cycles, 〉2 ng/mL 19 cycles; 716 cycles were used with down-regnlation long protocol, ≤ 1 ng/mL 588 cycles, 1-2 ng/mL 108 cycles, 〉2 ng/mL 20 cycles. [Results] There were no statistically differences in the number of oocytes retrieved, fetilization rates, embryos cleavage rates and high quality embryos rates among three groups. With the increase of progesterone level, implantation rates and pregnancy rates decreased, while the spontaneous abortion rates increased, but there were no significant differences. [Conclusions] In controlled ovarian hyperstimulation, the increase of progesterone level of late follicular phase has no significant effect on the implantation rate, clinical pregnancy rate, and the spontaneous abortion rate.
