用Dot-ELISA法检测SLE患者血清抗鸡卵核蛋白抗体的研究

摘　　要：[目的]采用斑点酶联免疫吸附试验(Dot-ELISA)法检测系统性红斑狼疮(SLE)患者血清中抗鸡卵核42000蛋白的抗体,探讨该抗体与SLE患者相关抗体检测指标的关系,以进一步研究该抗体在SLE患者发病过程中的作用。[方法]用Dot-ELISA法测定200例正常人、424例非SLE结缔组织疾病患者、232例SLE患者血清中的抗鸡卵核42000蛋白抗体及其滴度。[结果]检测200例正常对照人群未见阳性;96例硬皮病(PSS)患者、152例皮肌炎和多发性肌炎(DM/PM)患者、176例混合性结缔组织病(MCTD)患者的血清中末检出有交叉反应。232例SLE患者血清中有156例查出该抗体,阳性率为67.24%。SLE患者血清抗鸡卵核抗体的几何平均倒数滴度(GMRT)在患病的活动期为684.81±25.34,明显高于缓解期的404.53±16.63;该抗体与抗双链DNA抗体(抗ds-DNA抗体)滴度无显著相关性;与抗核抗体(ANA)滴度呈显著的正相关,与补体C3呈显著的负相关。[结论]抗鸡卵核42000蛋白抗体的检测,对SLE诊断和对病情活动程度的判断有一定的临床指导作用。[Objective] To detect the anti-42000 protein from egg nucleus antibody in systemic lupus erythematosus （SLE） patients with Dot Enzyme linked immunosorbent assay （Dot-ELISA）, so as to explore the relationship between the anti-42000 protein from egg nucleus antibody and the correlative antibodies in SLE patients and to study it＇s role in the course of disease occurrence. [Methoods] The serum levels of anti-42000 protien form egg nucleus antibody were measured by DotELISA in 200 healthy persons, 424 non-SLE connective tissue disease cases and 232 SLE patients. [ Results] Among 232 SLE cases, 156 cases were detected to be positive and the positive rate was 67.24%, but no cross reactions were found in sera from 200 normal individuals, 152 cases of dermatomyositis or polymyositis （DM/PM）, 96 cases of progressed systemic scleroderma （PSS） and 176 cases of mixed connective tissue disease （MCTD） . The geometric mean reciprocal titre （GMRT） of the anti-42000 protien from egg nucleus antibody in active SLE patients was 684.81 ± 25.34, which was significantly higher than that （404.53 ± 16.63） during remission. There was no obvious correlation between the GMRT and anti-d5-DNA titre. There was a positive correlation between the GMRT of the antibody and ANA titre. Whereas, there was a negative correlation between the GMRT and C3 concentration. [Conclusions] The detection of anti-42000 protein from egg nucleus antibody is useful to diagnose SLE and judge the activity of SLE.
