非洛地平对良性前列腺增生合并原发性高血压患者血管内皮功能的影响

摘　　要：目的:研究非洛地平对良性前列腺增生(BPH)合并原发性高血压(EH)患者血管内皮功能的影响。方法:55例BPH合并EH患者,口服非洛地平5mg,qd,治疗6wk,检测治疗前、后的血浆内皮素-1(ET-1)、一氧化氮(NO)、血管性假血友病因子(vWF)、肱动脉血流依赖性舒张功能(FMD)、最大尿流率、平均尿流率、国际前列腺症状评分(I-PSS),同时检测正常对照组(37名健康男性)ET-1、NO、vWF及FMD水平。结果:与正常对照组比较,治疗前ET-1、NO、vWF及FMD变化具有统计学差异(P<0·05),治疗后以上各指标变化无统计学差异(P>0·05);与治疗前比较,治疗后ET-1、NO、vWF及FMD变化有统计学差异(P<0·05),最大尿流率、平均尿流率、I-PSS变化无统计学差异。结论:改善血管内皮功能可能是非洛地平在降压治疗同时缓解BPH症状的作用机制之一。OBJECTIVE： To observe the effects of felodipine on blood vessel endothelial function in patients with benign prostatic hyperplasia（BPH） complicating essential hypereension（EH） . METHODS： Fifty five patients with BPH complicating EH were assigned to receive 5mg felodipine orally qd for 6 weeks, with levels of endothelin - I（ET- 1）, nitrous oxide（NO） and von Wilebrand factor（vWF）, brachial artery flow-mediated dilatation（FMD） function, the maximum urine flow rate, the average urine flow rate and International Prostate Symptom Score（I - PSS） determined before and after treatment. Meanwhile levels of ET 1, NO and vWF and FMD in 37 normal healthy males were determined. RESULTS： In the treatment group compared with normal control group there were significant differences in levels of ET- 1, NO and vWF and FMD（P 〈 0.05） before treatment, but which showed no significant difference in the two groups after treatment（P 〉0.05）. There were signif- icant differences in ET- 1, NO and vWF levels and FMD after treatment as compared with before treatment（P 〈 0.05）, but no significant difference was noted in maximum urine flow rate, average urine flow rate and I- PSS. CONCLUSION： The improvement of blood vessel endothelial function may be attributed to one of the mechanisms of felodipine that it can improves BPH syndromes meanwhile as a hypotensive agent.
