摘 要:目的:计算机辅助设计氨基糖苷类抗生素个体化给药方案。方法:以阿米卡星为例,根据患者的性别、年龄、身高、体重、血清肌苷浓度,计算血清肌苷清除率、药物清除率和消除速率常数,以药动学模型理论和药效学原理为依据,用Excel编写模拟计算程序。结果:只需输入患者生理参数,即可输出男性和女性静脉注射、血管外给药、间歇静脉滴注3种给药方式的给药剂量和稳态血药浓度。小儿剂量可由小儿体表面积与成人体表面积之比折算而成。且因Excel具有重复计算功能,改变输入参数值,可一步得到所有结果供临床参考。该方法适用于其它氨基糖苷类抗生素。结论:根据药动学参数和患者的生理特点,可利用Excel函数设计阿米卡星个体化给药方案,该法简单、可靠、直观。OBJECTIVE : To establish a simple computerassisted individualized dosage regimen design of aminoglycoside antibiotics.METHODS: Serum carnine clearance, drug clearance and elimination rate constant were computed based on patients' personal information such as gender, age, body height, body weight and serum carnine concentration;then analog computation program was programmed using Excel based on pharmacokinetics model theory and pharmacodynamic principles. RESULTS: Data on dosage and steady plasma-drug concentration by 3 different routes of administration(iv; im and iv gtt) were available after the input of patients' (male or female)physiological parameters .The dosage for children can be figured out from the ratio of children's body surface area(BSA)to adults' .Moreover, because of the repeated calculation function of Excel, the corresponding data could be obtained for clinical references once the new parameter value was inputted.This method is applicable for other aminoglycoside antibiotics.CONCLUSION:An individualized dosage regimen of aminoglycoside antibiotics can be designed by means of Excel function based on patients' pharmacokinetic parameters and physiological parameters, and this method is simple, reliable and directly perceived through the senses.
