西咪替丁对体外循环手术患者免疫功能的保护作用

摘　　要：目的 探讨西咪替丁对体外循环心内直视手术患者免疫功能的保护作用.方法 选取2004年1月～2005年1月在我院行心瓣膜置换术的30例风湿性心脏瓣膜病患者分为两组,西咪替丁组麻醉后切皮前至术后第5d静脉滴注西咪替丁 400 mg;对照组静脉滴注维生素C 2.0 g,并于术前1 d、术后1、3、5、7和14 d观察西咪替丁对术后患者的细胞免疫、体液免疫及红细胞免疫的影响.结果 西咪替丁组患者术后CD3、CD4、CD4/CD8、NK细胞活性、白细胞介素-2（IL-2）、RBC-C3bRR及RBC-ICR等指标下降程度明显低于对照组（P＜0.01）,术后3～5 d开始回升,7 d恢复至术前水平（P＞0.05）;对照组于术后7 d开始回升,14 d恢复至术前水平.结论 西咪替丁对体外循环心脏直视手术患者的免疫功能有明显的保护作用.Objective To investigate the protection effects of cimetidine for the immune function of patients underwent cardiac operation under cardiopulmonary bypass （CPB）. Methods From Jan. 2004 to Jan. 2005, thirty patients suffered from rheumatic cardiac valvular disease received cardiac valve replacement in our hospital, and were divided into cimetidine group and control group. The effects of cimetidine on cellular immune, fluid immune and erythrocytic immune were observed ld before operation, 1, 3, 5, 7 and 14d after operation. Results After operation, CD3, CD4, CD4/CD8, NK cell activity, Interleukin-2 （IL-2）, RBC-C3bRR and RBC-ICR in cimetidine group were significantly higher than those in the control group（P〈0. 01）. In cimetidine group,those index began to recover on the postoperative 3 to 5 days, and return to normal level on the postoperative 7 days （P〉0. 05）. In control group, 7 and 14 days respectively. Conclusion The protective effects of cimetidine on immune function of open-heart operative patients are significant.
