尿毒清胶囊延缓慢性肾功能衰竭患者透析治疗的临床观察

摘　　要：目的:观察尿毒清胶囊治疗慢性肾功能衰竭的临床疗效,重点观察其延缓肾衰患者透析治疗的作用。方法:将78例患者随机分为2组各39例。治疗组在基础治疗同时加尿毒清胶囊(由附子、黄芪、大黄、法半夏、土茯苓等组成)治疗;对照组在基础治疗同时加包醛氧淀粉治疗。观察患者主要症状、中医证候和实验室检查指标[血红蛋白(Hb)、红细胞总数(RBC)、血清肌酐(Cr)、尿素氮(BUN)、内生肌酐清除率(CCr)、血清白蛋白(Alb)、血清总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)]等。结果:治疗后治疗组主要症状总积分、大部分症状单项积分均少于对照组,2组比较,差异有显著性或非常显著性意义(P<0．05,P<0．01);治疗组症状有效率和总体疗效均高于对照组,2组比较,差异有显著性或非常显著性意义(P<0．05,P<0．01);治疗组治疗后能提高患者血清Alb、HDL-C、CCr,改善TC、TG、BUN、Cr等,还能减少同期内需要透析的病例数,2组比较,差异有显著性或非常显著性意义(P <0．05,P<0．01);对RBC、Hb的升高作用,2组比较,差异无显著性意义(P>0．05)。结论:尿毒清胶囊不仅能改善慢性肾功能衰竭患者临床症状,而且能改善肾功能,延缓尿毒症患者接受透析治疗的时间。Objective: To observe the clinical effect of Niaoduqing capsule (NC,尿毒清胶囊) for postponing the development of chronic renal failure (RF) after dialysis therapy. Methods: 78 cases were allocated to 2 groups (each group 39 cases) randomly. The treatment group was treated by basic treatment added with NC (composed of Radix Aconiti Lateralis Preparata,Radix Astragali, Rhizome Pinelliae Preparata and Rhizoma Smilacis Glabrae, etc.); the control group by added with coated aldehyde oxystarch. The chief symptoms, syndrome pattern and laboratory parameters such as Hb, RBC, Cr, BUN, CCr, Alb, TC, TG, LDL - C and HDL - C were observed. Results: In treatment group, the total scoring of chief symptoms and scoring of most individual symptoms were lower than that of control group (P<0.05, P<0.01). Effective rate on symptoms and total curative effect were also higher than that of control group (P<0.05, P<0.01). After treatment, Alb and HDL - C、CCr were increased, TC、TC、BUN、Cr、CCr、etc. were decreased and the cases necessary for dialysis were reduced in treatment group showed significant (P<0.05,P<0.01) as compared with control group. The increase of RBC and Hb showed no significant difference between both groups (P>0.05). Conclusion: NC not only alleviates the clinical symptoms of CRF but also improves the renal function and postpones dialysis therapy in cases of uremia
