坐位神经外科手术麻醉及围术期麻醉管理的探讨

摘　　要：目的:探讨神经外科手术中坐位全身麻醉对循环血流动力学影响及围麻醉期的管理。方法:50例后颅窝手术病人,采用静脉复合诱导,吸入异氟醚伍用异丙酚维持麻醉,术中监测血流动力学,分别在麻醉前、坐位前、坐位后及术毕平卧位等时间点进行测定。结果:体位改变后出现血压下降,心率增快(P<0.05),术毕平卧位恢复至麻醉前水平。结论:神经外科病人在坐位全麻手术期应维持血流动力学平稳,重视围麻醉期的管理。Objective： To investiage the influence of sitting position during general anesthesia in systemic hemodynamics and the management during anesthetic stage. Methods： 50 Patients undergoing posterior fossa surgery were anesthe tized with isoflurance - propofol pancuronium. The hemodynamics was monitored before anesthesia, before and aften sitting, and at prostation after operation. Results： The blood pressure decreased and the heart rate increased after body position （P（0.05）. The blood pressure and the heart rate at prostration af- ter operation returned the values before anesthesia, Conclusion： The hemodynamics should maintain stedily in patients on sitting position during general anesthsia for neurosurgery. The management during anesthesia should also be emphasized.
