摘 要:目的探讨社区中代谢综合征(MS)患者血清游离脂肪酸谱特征及与其他糖脂代谢参数间关系。方法采用国际糖尿病联盟 MS 的诊断标准,用气相色谱-质谱方法测定社区居民中158例含 MS 不同组分患者[MS 患者61例(MS 组),MS 高危者97例(MS 高危组)]与43例体检正常者(对照组)血清游离脂肪酸组分。结果 MS 组多不饱和脂肪酸、n6系多不饱和脂肪酸浓度等高于 MS高危与对照组(P<0.01或 P<0.05)。MS 组中有糖尿病与无糖尿病者相比,血清游离脂肪酸各组分浓度差异均无统计学意义(P>0.05)。MS 高危组中,有糖尿病者业油酸、n6系多不饱和脂肪酸、总游离脂肪酸(TFA)浓度等高于无糖尿病者,饱和脂肪酸/总脂肪酸值低于无糖尿病者(P<0.05或 P<0.01)。3组单不饱和脂肪酸/总脂肪酸值的均值为0.15左右。TFA、多不饱和脂肪酸与体重指数、平均血压、空腹血糖、甘油三酯呈正相关(r=0.164和0.211,0.217和0.287,0.198和0.275,0.266和0.158,P<0.05或 P<0.01)。结论游离脂肪酸与 MS 多种组分相关。TFA、n6系多不饱和脂肪酸升高,单不饱和脂肪酸缺乏,可能在其发病中起重要作用。Objective To explore the relationship between free fatty acid (FFA) composition and other glucose and lipid metabolic parameters in patients with metabolic syndrome (MS) in community. Methods Semm FFA profile was measured with gas chromatography and mass spectrometry in 158 patients with varied metabolic syndrome components (MSC), including 61 with MS and 97 at high-risk for MS, and 43 control subjects, with diagnostic criteria by the International Diabetes Federation (IDF). Results Patients with MS had higher parameters of polyunsaturated fatty acid (PUFA) and n6PUFA, as compared to the patients at high risk and normal subjects (P 〈 0.01, P 〈 0. 05). There was no significant difference in the above mentioned FFA composition between the patients of MS with or without diabetes ( P 〉 0. 05 ). Among the high-risk group, those with diabetes had increased linoleic acid, n6PUFA and total fatty acid (TFA), and decreased saturated fatty acid (SFA)/TFA, as compared to those without diabetes (P 〈 0. 05, P 〈 0. 01 ). Their mean level of monounsaturated fatty acid (MUFA) / TFA was 0. 15 or so. TFA and PUFA correlated remarkably with body mass index, mean arterial pressure, fasting blood glucose and triglycerides (r = 0. 164 and 0. 211,0. 217 and 0. 287,0. 198 and 0. 275,0. 266 and 0. 158, respectively, P 〈 0.05, P 〈 0. 01, respectively) in all the subjects. Conclusions Semm level of FFA associated with varied MSC. Increased level of TFA and n6PUFA, and decreased MUFA may play a role in pathogenesis of MS.
