前列地尔脂微球载体制剂注射液配合血浆置换治疗重型肝炎临床研究

摘　　要：目的:评价前列地尔脂微球载体制剂注射液(Lipo-PGE1)配合血浆置换治疗重型肝炎的疗效。方法:将102例重型肝炎患者随机分为试验组和对照组。试验组53例(亚急性重型肝炎6例,慢性重型肝炎47例)在内科综合治疗基础上加用血浆置换及Lipo-PGE1;对照组49例(亚急性重型肝炎3例,慢性重型肝炎46例)仅用内科综合治疗加用血浆置换。测定两组患者治疗前后血清总胆红素(TBIL)、谷丙转氨酶(ALT)、白细胞介素-6(IL-6)、肿瘤坏死因子(TNF-α)和内毒素浓度。结果:试验组和对照组治疗后血清TBIL,ALT,IL-6,TNF-α和内毒素较治疗前均明显降低(P<0.05或P<0.01);其中试验组IL-6和TNF-α还显著低于对照组[IL-6:(28.5±18.4)vs.(38.9±12.4)ng.L-1,P<0.01;TNF-α:(168.46±10.24)vs.(258.39±11.42)ng.L-1,P<0.01]。试验组总有效率(治愈及好转)为73.6%,显著优于对照组46.9%(P<0.01)。结论:Lipo-PGE1配合血浆置换是治疗重型肝炎的有效方法。Objective：To evaluate the efficacy of Lipo-PGE1 combined with plasma exchange in treatment of patients with severe hepatitis. Methods： One hundred and two patients with severe hepatitis were randomly received with either comprehensive internal medical treatment combined with plasma exchange and Lipo-PGE1 （n = 53, of which 6 with subacute severe hepatitis and 47 with chronic severe hep- atitis） or comprehensive internal medicine treatment plus plasma exchange only （n = 49, of which 3 with subacute severe hepatitis and 46 with chronic severe hepatitis）. The efficacy of both therapies was as- sessed based on the status of TBIL, ALT, IL-6, TNF-α and endotoxin pre- and post-treatment. Results： The patients experienced significant reductions of levels of TBIL, ALT and endotoxin post-therapy than pre-therapy on both groups （P 〈 0.05）. The Lipo-PGE1-added patients had levels of the IL-6 and TNF- ct of （28.5±18.4） ng.L^-1 and （ 168.46±10.24） ng.L^-1 , compared to the patients treated without Li- po-PGE1 [ （38.9±12. 4） ng. L^-1 and （258.39±11.42） ng.L^-1] post-therapy. The total effect rate （cured and improved） of tri-therapy and bi-therapy was 73.6% vs. 46. 9% （P 〈0. 01）. Conclusion：Li- po-PGE1 combined with plasma exchange offered a more effective therapy for patients with severe hepatitis.
