摘  要:目的探讨重症急性胰腺炎(SAP)时血内皮素(ET)、一氧化氮(NO)的变化及其相互关系,以及参附注射液的干预作用,为临床采用中西医结合治疗SAP提供依据。方法将大鼠分为假手术组、SAP组和参附组,检测各组中不同时段(6、12、24h)血ET、NO的水平,并观察胰腺组织病理学改变。结果与假手术组相比,SAP组随病变进展ET、NO从两者平行升高逐渐转向ET单一升高,致ET/NO值上升,胰腺病理评分明显增高;参附组血浆ET水平低于SAP组,而NO总体水平与SAP组比较差别无显著性,ET/NO比值基本与假手术组一致,胰腺病理评分明显低于SAP组。结论参附注射液可下调ET水平,维持ET与NO平衡,对SAP病变有保护作用。Objective: To explore the etiological role of serum endothlin(ET) and nitric oxide(NO) in the severe acute pancreatitis(SAP), and to study the influences of Shenfu Injection(SFI, 参附注射液) on them. Methods: Rats were divided into the fame operated group, the SAP group and the SFI group. The changes of the levels of ET and NO at varying time -points(the 6th, 12th, 24th hour after treatment) were detected, and the pathologic change of the pancreas was studied by light microscope. Results: Compared with the fame operated groups, the levels of SAP group increased according as exacerbation of SAP. The levels increased in parallel at the beginning and then only endothelin increased, leading to an increased endothelin/ nitric oxide ratio. The histopathologic score of pancreatitis was increased significantly. The level of ET of SFI group was significantly lower than that of the SAP group, while the level of NO was similar to that of the SAP group. The histopathologic score of pancreatifs in SFI group was lower significantly than in the SAP group. Conclusion: SFI can reduce level of ET levels, maintain the balance between endothelin and nitric oxide, and have protective effect to the damage from acute pancreatitis in rats.
