有氧运动对高血压并发房颤患者血小板表面糖蛋白受体的影响

摘　　要：目的:探讨规律运动对高血压并发房颤患者血小板表面活化分子标志蛋白及血小板聚集率的影响。方法:将我院门诊就诊的高血压并发房颤患者61例随机分为两组,常规治疗组(对照组)31例,规律运动组(实验组)30例,后者在常规治疗的基础上采取中等量的运动疗法3个月。采用流式细胞术以单克隆抗体分子作为探针,分别测定两组患者运动前后血小板膜上CD62P、CD61的阳性百分率及血小板聚集率。结果:进行运动疗法3个月后,对照组与实验组两组患者,收缩压(mmHg)分别为158±3.8及151±4.3(P<0.05),舒张压(mmHg)分别为98±3.4及90±3.1(P<0.01);左室射血分数(%)分别为54.6±4.7及60.3±3.6(P<0.05);血小板膜上CD62P阳性百分率(%)分别为27.4±2.2及20.5±3.1(P<0.05)、CD61阳性百分率(%)分别为27.2±3.5及21.3±2.9(P<0.05);血小板聚集率(%)分别为81.2±4.3及73.6±4.6(P<0.05)。结论:规律运动能降低高血压并发房颤患者血小板表面活化受体分子的密度及血小板聚集率,有利于改善高血压患者的血栓前状态。Objective To explore the effects of aerobic exercise on platelet membrane glycoproteins in the patients with hypertension complicated with atrial fibrillation. Methods Sixty-one hipertensive patients with atrial fibrillation were randomly divided into exercise group （n = 30） and control group （n = 31 ）. The exercise with moderate intensity was 30 min persession, 5 sessions perweek for 3 months. The platelet membrane glycoprotein CD62P, CD61 （m a） were determined by flow cytometric analysis and platelet aggregating ratio was measured before and after regular aerobic exercise. Results After three months of aerobic exercise, the blood pressure （mmHg） （SBP： 158 ± 3.8 vs 151 ± 4.3, P 〈 0.05; DBP： 98 ± 3.4 vs 90 ± 3.1, P 〈 0.01 ）, the positive percent of platelet membrane glycoproten CD62P（%） （27.4 ± 2.2 vs 20.5 ± 3.1, P 〈 0.05）, CD61（Ⅲa）（%） （27.2 ± 3.5 vs 21.3 ± 2.9, P 〈 0.05） and platelet aggregating ratio（%） （81.2 ± 4.3 vs 73.6 ± 4.6, P 〈 0.05） in aerobic exercise group were significantly decreased compared with those of control group, except LVEF （%）（54.6±4.7 vs 60.3±3.6, P 〈 0.05）. Conclusion Regular aerobic exercise can decrease the positive percent of platelet membrane glycoprotein and platelet aggregating ratio in the atrial fibrillation patients complicated with hypertension.
