食管癌切除术后血清微量元素及其运载蛋白的应激变化

摘　　要：目的:探讨血清铜、锌、铁及其转运蛋白在食管癌切除术后的变化情况。方法:对9名男性和17名女性食管癌患者手术前后抽取血清进行铜、锌、铁及其转运蛋白测定。结果:血清铜、锌、铁在手术初期呈相似变化,即先增加再降低,呈现不同的应激变化。其中锌和铁变化有显著性差异,而血清铜水平变化均无显著性差异(P>0.05);3种转运蛋白均自手术开始时下降,其中白蛋白和转铁蛋白在整个研究期均下降,且有明显差异;而铜蓝蛋白的变化则相应较慢,至第7天才出现显著增高(P<0.05)。结论:外科手术时机体内微量元素的变化极为复杂,对进行大外科手术治疗的患者给予补充一定量的微量元素益于其疾病恢复。Objectibe：To observe the changes on the concentrations of iron, copper, and zinc and of their transport proteins in serum after surgery in patients with esophageal carcinoma. Methods： A total of 26 patients had carcinoma of esophagus （9 male and 17 female, ages 43--70 years, mean 53.6 years）, their venous blood was drawn at pro- operative and post- operative different time points. Serum concentrations of iron, copper, zinc and of their respective transport proteins transferrin, albumin and ceruloplasemin were determined. Results：The pattern of changes of iron, copper and zinc was similar, with an early increase, then a decrease subsequent a their respective stress changes in the concentrations. Iron and zinc of these were significant. However, the changes of copper was no significant（P 〉 0.05）. The decrease in concentrations of transefrrin and albumin were statistically significant in this study period, whereas the ceruloplasemin concentration changes slowly in early postoperative period until the seventh day with an increase significantly（ P 〈 0.05 ）. Conclusions ： The results indicated that changes of trace dements in serum is complexity after major surgery, which suggest that it is beneficial to increase patients nutrition and to help patients recovery if they were fed some essential trace dements during the post - operative period.
