摘 要:目的探讨以腺病毒载体局部应用血管内皮生长因子(Ad-VEGF)和碱性成纤维细胞生长因子(Ad-bFGF)基因治疗对大鼠缺血皮瓣生存的影响。方法50只SD大鼠随机分成5组,每组10只。在其背部形成8cm×2cm的缺血皮瓣。于皮瓣远端皮下分别注射Ad-VEGF和Ad-bFGF(目的基因组),Ad-VEGF(基因对照组),Ad-bFGF(基因对照组)Ad-GFP(绿色荧光蛋白基因对照组),PBS(磷酸盐缓冲液空白对照组)。48h后按原设计掀起皮瓣并原位缝合,术后7d,测量皮瓣的成活面积及免疫组化,组织学检查等。结果目的基因组皮瓣成活面积比及单位面积微血管密度与其它4个对照组有显著性差异。免疫组化目的基因组毛细血管周围VEGF、bFGF沉积。结论以腺病毒为载体局部应用VEGF和bFGF基因能增加缺血皮瓣的成活面积。Objective To investigate the effect of adenovirus-mediated gene therapy with vascular endothelial growth factor (VEGF)and basic fibroblast growth factor (bFGF)delivered into the subdermal space to treat compromised skin flaps in rats. Methods Fifty SD rats were randomized into five groups with 10 rats each. An ischemic skin flaps of 8 cm× 2 cm were designed on the back. Rats received subdermal injection of Ad-VEG and Ad-bFGF(group A), Ad-VEGF(group B), Ad-bFGF (group C)or Ad-GFP(group D). Another set of animals receiving PBS was designated as the control (group E). Two days after treatment, the flap was elevated as original designe and sutured back to its bed. The survival rate of the flap was evaluated on the 7th day after operation. Results The viable rate of the flap and dermovascular densities in Ad-VEG and Ad-bFGF group was signifficantly different from that in the other four grops on the 7th postoperative day. Immunohistochemical staining showed that VEGF and bFGF were expressed in the survival flap injected with Ad-VEGF and Ad-bFGF. Conclusion Local application of adenovirus-mediated VEGF and bFGF may efficiently improve the survival of ischemic skin flap.
