摘  要:目的用^(99)Tc^m-DTPA 示踪探讨电化学治疗中脉冲电场致活体骨骼肌电穿孔的最适剂量学参数。方法以32只健康兔双后肢骨骼肌为研究对象,均于耳缘静脉"弹丸"式注射^(99)Tc^m-DTPA,之后梯度改变脉冲电场参数,处理所有兔左侧骨骼肌,建立电化学治疗的模型,右侧自身对照不予处理。用γ显像及感兴趣区(ROI)技术,分析电击处骨骼肌局部放射性浓聚及变化。结果左侧电击局部放射性明显浓聚,程度大于其邻近及右侧对照区域,且浓聚量与脉冲各参数值呈一定的相关性。电压峰值在80~100 V 时,放射性摄取随电压升高而迅速增加;>100~180 V 时,放射性摄取随电压升高而缓慢增加;>180 V 时随电压升高放射性摄取减少;至250 V 时,放射性摄取不再减少。频率在20~100 Hz 时,放射性摄取随频率增大而迅速增加;>100~800 Hz 时,放射性摄取随频率增大而减少。电容在0.01~0.033 μF时,放射性摄取随电容增大而迅速增加;>0.033~0.33μF时,放射性摄取随电容增大而减少。结论脉冲电压峰值80~180 V、频率20~100 Hz、电容0.01~0.033 μF为兔骨骼肌电化学治疗的脉冲电场最适参数范围,均能独立影响电穿孔,这可为临床肿瘤电化学治疗等提供参考。Objective Pulsed electric fields (PEFs) could be used as an electrochemotherapy for malignant diseases, but the optimal electroperforation parameters on normal tissues remains to be determined. In this study, a dynamic ^99Tc^m-DTPA imaging was used to study the effect of PEFs of different parameters since accumulation of ^99Tc^m-DTPA was found related to the extent of tissue destruction. Methods Thirty-two healthy rabbits were randomly grouped as their left hind extremities being treated with PEFs of different peak electric voltages (80, 100, 150, 180, 200 and 250 V) , or frequencies (20, 50, 100, 300 and 800 Hz), or capacitance (0.01, 0.022, 0.033, 0.1 and 0.33 μF). The right extremities served as controls. As PEF treatment went on, dynamic ^99Tc^m-DTPA imaging over both extremities using gamma camera was carried out. Region of interest (ROI) was applied over each extremity for comparative study of the focal radioactivity in light of different PEFs' parameters. Results In all animals, the accumulation of ^99Tc^m-DTPA at the PEFs treated regions was significantly higher than that of the controls. The focal radioactivity steadily increased with higher electric voltage within 80 to 180 V, but reversely related as the voltage over 180 V. The focal radioactivity also increased as the electric frequency increased in a range from 20 to 100 Hz, but higher frequency( 100 - 800 Hz) reduced the focal radioactivity. In low electric capacitance (0.01 -0. 033 μF), the regional ^99Tc^m-DTPA accumulation correlated with capacitance increment; but was unrelated in higher capacitance (0. 033 ~ 0.33 μF). Conclusion Dynamic ^99Tc^m-DTPA imaging data suggested that the optimal parameters of PEFs upon skeletal muscle would be as voltage between 80 ~ 180 V, frequency between 20 ~100 Hz and capacitance between 0.01 ~0. 033 μF.
