摘  要:目的分析尿路感染大肠埃希菌对抗生素的耐药情况,为临床治疗和研究提供参考。方法回顾分析我院近三年多自临床尿标本分离的605株大肠埃希菌的耐药情况,细菌的鉴定及药敏采用Microscan Walkaway 40全自动系统。结果17.7%的大肠埃希菌产超广谱β-内酰胺酶(ESBLs)。大肠埃希菌对哌拉西林/他唑巴坦、亚胺培南、阿米卡星和三四代头孢菌素等的耐药率相对较低(<25%),对氨苄西林、哌拉西林、环丙沙星和复方磺胺甲唑的耐药率较高,分别为90.1%、85.5%、72.2%和72.2%。三年来大肠杆菌对氨曲南、头孢他啶的耐药率呈逐年升高的趋势(χ2分别为12.5及12.1,P<0.05)。结论细菌耐药性监测对指导临床合理用药具有重要意义。Objective: To investigate the antimicrobial resistance patterns of Escherichia coli causing urinary tract infections (UTI) so as to provide some evidence for clinical therapy. Methods: The susceptibility of antimicrobial agents in 605 strains of E. coli were collected retrospectively in our hospital from January in 2002 to June in 2005. Identification and susceptibility of the pathogens were performed by Microscan Walkaway 40 Automatic System. Results: Isolated from urine 17.7% of E. Coli were ESBL producing strains. The drug resistance of E. coli to Piperacillin/Tazobactam, Imipenem, Amikacin and the 3rd, 4th generation of Cephalosporin was less than 25 %, while the pathogen show high resistance to Ampicillin, Piperacillin, Ciprofloxacin and Trimethoprirn/Sulfamethoxazole (90.1%, 85. 5%, 72.2% and 72.2%, respectively). During last 3 years there was increasing trend of resistance to Aztreonarn, Ceftazidime (χ2 = 12.5 and 12.1, P 〈 0.05). Conclusion: Surveillance would be heloful for oroviding evidence for ohvsicians in empiric treatment of UTI.
