抗幽门螺杆菌治疗前后结节性胃炎和十二指肠溃疡的流行病学

摘　　要：目的了解抗幽门螺杆菌（Hp）治疗前后结节性胃炎和十二指肠溃疡（DU）胃镜检出情况和流行病学特点。方法对本院1996~2005年1275例胃镜受检病例（男671例,女60例;年龄1~16岁）,以开展抗Hp治疗为界,分析胃镜检出情况,并进行流行病学分析。结果抗Hp治疗后DU的检出率（11.89%）较抗Hp治疗前（17%）明显下降（P〈0.05）,但1~5岁组DU的检出率却显著升高。抗Hp治疗后结节性胃炎的检出率（8.08%）却较抗Hp治疗前（2.17%）显著升高（P〈0.05）。结节性胃炎的发病平均年龄[（9.54±3.0）岁]小于DU的发病平均年龄[（10.49±2.61）岁]（P〈0.05）。抗Hp治疗前后DU和结节性胃炎的检出率也随年龄增加而递增,但在年龄分布、性别比例上无明显差异。结论Hp感染与DU和结节性胃炎密切相关,抗Hp治疗改变DU的自然病程,可使炎性反应改变、消退,加速溃疡愈合,降低溃疡复发。结节性胃炎的检出率却较抗Hp治疗前升高,可能与目前抗Hp治疗指征、依从性及再感染有关。Objective To investigate the prevalence of nodular gastritis, duodenal ulcer before and after eradicative treatment of helicobactar pylori （Hp） infection in children with gastroscopy. Methods This was a retrospective analysis of 1 275 children,age ranging 1 to 16 years old, collected from our hospital in recent l 0 years, detecting rates of before eradicative treatment and after eradicative treatment were analyzed. Gastroscopes were analyzed with regard to a possible association with the infection. Results The detecting rate（ 11.89% ） of duodenal ulcer after eradicative treatment was lower than that（ 17% ） before eradicative treatment（ P 〈 0.05 ）, but increased in the age group of 1 - 5 years. The detecting rate（ 8.08% ） of nodular gastritis after eradicative treatment were higher than that（ 2.17% ） before eradicative treatment（ P 〈 0.05 ）. The median age of nodular gastritis [ （ 9.54 ± 3.0） years ] was lower than that of duodenal ulcer [ （ 10.49 ± 2.61 ） years ] （ P 〈 0.05 ）. The detecting rates of nodular gastritis and duodenal ulcer increased with age before and after eradicative treatment, but there was no significant differ- ence in age distribution and sex rate. Conclusions Hp is significantly linked to duodenal ulcer and nodular gastritis, while eradicative treatment of Hp can change the natural course of duodenal ulcer, accelerate cicatrisation, and decrease recurrence. Increased detecting rates of nodular gastritis after eradicative treatment are associated with treatment guide, compliance and possible reinfection.
