摘  要:目的探讨婴儿期母乳喂养与学龄期儿童认知和行为发育的关系。方法选取泰安市宁阳县城区二所小学的581名7~11岁儿童作为研究对象。采用瑞文标准推理测验、Achenbach儿童行为量表(CBCL)和自制儿童认知和行为影响因素问卷对其学龄期认知、行为发育情况和婴儿期喂养状况等进行调查,分析婴儿期母乳喂养类型和母乳喂养持续时间与学龄期儿童认知和行为发育之间的关系。结果单因素分析发现4个月内仅喂母乳男童的知觉辨别、类同比较和抽象推理的正确题数多于对照组(Pa〈0.05),仅喂母乳女童的类同比较、比较推理、系列关系的正确题数多于对照组(Pa〈0.05);多因素Logistic回归分析发现4个月内母乳喂养量越多、婴儿期母乳喂养持续时间越长,学龄期智商数越高,而行为总分越低。结论婴儿期母乳喂养量越多、持续时间越长,学龄期认知发育越好,行为问题发生越少。Objective To explore the association between infant breasffeeding and cognition and behavior development in school age children. Methods Five hundred and eighty - one children aged 7 - 11 years old from two schools in Ningyang of Tai'an. Their cognition and behavior development during school age were assessed by Ravin's standard progressive matrices and child behavior check list(CBCL). Their feeding conditions during infant period, family environment, education level of parents,et al,were investigated with self- made inventory questionnaire. Association between infant breasffeeding types, duration of breastfeeding and cognition , behavior development were analyzed. Results The single factor analysis showed the scores of perceptual distinction, similarity, and abstract thought in children who were only breast fed before 4 months were higher than those of controls for male children( Pa 〈0.05 ). The scores of similarity, comparison thought and series relation in children who were only breast fed before 4 months were higher than those of controls for female children( Pa〈 0.05 ). The multiple factors Logistic regression analysis showed the more breastfeeding during the first 4 months and the longer the duration of breastfeeding, the higher scores of cognition and less behavior problems during school period. Conclusion Infants who are fed breast milk more than infant formula, or who are breasted for longer periods, have a better cognition development and a lower risk of behavior problems occurrence during school age.
