回收式自体输血的临床应用体会

摘　　要：目的回顾我院回收式自体输血的术中应用及患者术后恢复情况,评价回收式自体输血的安全性和有效性。方法回顾725例术中应用回收式自体输血的患者回收、回输血液量、术后输注异体血以及治愈情况。比较其中未输异体血的140例回收式自体输血患者术前、术后1d和术后5d的红细胞计数(RBC)、血红蛋白(Hb)、红细胞比容(Hct)、白细胞计数(WBC)、血小板计数(PLT)。结果经血液回收机回收血液总量1501920ml,平均每例2072ml。725例患者中,死亡7例术后,其余患者均康复出院。术后1d与术前比较RBC、Hb、Hct、PLT、WBC间差别有显著性意义(P<0.05);术后5d与术前各指标间差别无显著性意义(P>0.05)。结论回收式自体输血可以减少异体血的输注,缓解血源紧张,降低输血并发症及医疗费用,在抢救大出血时发挥着积极作用。Objective To evaluate the safety and effectiveness of intraoperative cell salvage and autotransfusion of washed salvaged red blood cells. Methods Seven hundred and twenty five patients undergoing recovered self - blood transfusion were reviewed and statisticsed. Among them, RBC, Hb, Hct, PLT and WBC before operation, I day postoperatively and 5 day postoperatively were studied in 140 patients with no allogeneic transfusion. Results The volume of recovered blood is 1 5101 920 ml, the average is 2072 ml per people. Among 725 patients, 7 patients died postoperatively, other patients recovered and were discharged. Compared with before operation, RBC, Hb, Hct and PLT were lower at I day postoperatively, WBC rised （ P 〈 0. 05 ）, They recovered at 5 day postoperatively （ P 〉 0. 05 ）. Conclusion The use of intraoperative blood cell salvage significantly lowered the allogeneic transfusion requirement, which can lower the complications of allogeneic transfusion and lead to substantial cost savings.
