Castleman病继发肾淀粉样变性一例报告及文献复习

摘　　要：目的 报道1例罕见的Castleman病继发肾淀粉样变性,并进行相关文献复习。方法与结果 1例43岁男性病例,3年前因水肿、蛋白尿住院,经肾活检病理证实为AA型肾淀粉样变性,住院期间发现纵隔淋巴结肿大、血液及骨髓异常。出院后追踪观察,3年后因体表多部位淋巴结肿大而再次住院。经免疫学及淋巴结活检病理检查证实为多中心性浆细胞型Castleman病,与文献报道相符。结论 Castleman病可继发AA型肾淀粉样变性,其发病可能与Castleman病导致的免疫异常有关。Objective To report a rare case of renal amyloidosis secondary to Castleman＇s disease, and to review literatures concerned. Methods and Results A 43-year-old man, who was followed up for three years, presented proteinuria, anemia and poly-site enlarged lymph nodes. Renal amyloidosis of AA type was diagnosed by pathological examination of renal biopsy samples, and Castleman＇s disease of muhicentric and plasma cells type was diagnosed by immunology and pathological examination of lymph node biopsy samples. Conclusion Castleman＇s disease can result in renal amyloidosis, which pathogenic mechanism may be associated with the abnormal immune states caused by Castleman＇s disease.
