儿童幽门螺杆菌感染与缺铁性贫血的相关性

摘　　要：目的探讨儿童中幽门螺杆菌(Hp)感染与缺铁性贫血(IDA)的相关性。方法以因慢性腹痛而行胃镜检查及Hp检测的60例患儿为观察对象,根据检查结果分为Hp阳性慢性胃炎组、Hp阴性慢性胃炎组及Hp阳性胃部镜下无损害组三组。所有病例均检测血红蛋白(Hb)、平均红细胞体积(MCV)、血清铁(SI)、血清铁蛋白(SF)、总铁结合力(TIBC)等IDA指标。比较三组之间的IDA指标和IDA伴发率。结果三组病例IDA指标两两比较显示,Hp阴性慢性胃炎组分别和Hp阳性慢性胃炎组、Hp阳性胃部镜下无损害组的IDA指标比较差异均有统计学意义(P<0.01,P<0.05),而Hp阳性慢性胃炎组与Hp阳性胃部镜下无损害组之间的比较差异则无统计学意义(P>0.05)。Hp阴性慢性胃炎的IDA伴发率均明显低于Hp阳性慢性胃炎组和阳性胃部镜下无损害组(P<0.05),Hp阳性慢性胃炎组与Hp阳性胃部镜下无损害组之间的IDA伴发率差异无统计学意义(P>0.05)。结论儿童幽门螺杆菌感染与缺铁性盆血两者之间具有显著的相关性,其相关性较慢性胃炎更为密切。Objective To study the relationship between Helicobacter pylori （Hp） infection and iron deficiency anemia in children. Methods Make observation in children who had undertaken gastroscopy and Hp test because of chronic stomachache. According to the examination results, all the patients were divided into 3 groups which were group 1 ： Hp positive chronic gastrits group; group 2 ： Hp negative chronic gastritis group, and group 3 ： Hp positive without gastric lesion group. Iron deficiency anemia （IDA） indexes such as bemogloity（TIBC） were checked in all the groups. IDA indexes and related IDA incidence were compared among the three groups. Results After comparing IDA indexes between groups, it showed that there were obvious difference （P 〈 0. 01, P 〈0. 05） between group 2 and 1 ,group 2 and 3 respectively. There was no obvious difference（ P 〈0. 05） between group 1 and 3. The incidence of IDA in grup 2 was obvious lower（ P 〈0. 05） than that either in group 1 or 2. There was no obvious difference（ P 〈 0. 05） between group 1 and 3. Conclusion There is obvious relationship between Hp infection and iron deficiency anemia in children. Its correlation is closer than chronic gastritis.
