22例全髋关节翻修术中对髋臼骨缺损的处理

摘　　要：目的探讨全髋关节翻修术中髋臼骨缺损的处理经验。方法回顾分析22例全髋关节翻修术中对髋臼骨缺损的处理经验,研究植骨、固定、髋臼置换和功能康复的新方法。结果本组手术全部成功,术后无感染发生。随访6-58个月,平均22.6个月,随访时关节无疼痛,对手术结果满意,髋关节功能较术前显著改善。结论（1）王爱民提出的新的骨缺损分类法简单清晰,能更好的指导髋臼重建手术。（2）对节段性和混合性骨缺损大块植骨时应辅以颗粒骨,并尽量选用较稳固的钉板系统固定。（3）使用带缝匠肌髂骨瓣植入重建髋臼缺损为活骨移植,成骨较好。Objective To discuss the early experience of the management for acetabular defects in the revision of the total hip arthroplasty. Methods Twenty-two cases of acetabular defects in the revision of the total hip arthroplasty were retrospectively analyzed to research the new measures of bone grafts,fixation,acetabular reconstruction and functional rehabilitation, Results The operations were successful in 22 cases without any infection. The mean time of fellow-up was 22. 6 months with a range of 6-58 months. The function was good. Conclusion （1）The new classification system developed by professor Wang Aimin is simple and can guide the management of acetabular reconstruction. （2）The good result is from the application of only morselized bone or suipplement with structure bone autografting. It is better to fix structure bone by the plate and screw system. （3）The pedicle sartorius muscle iliac bone grafting is the first application for acetabtllar defects in total hip arthroplasty in 3 cases. The new bone formation was better.
